Postoperative Questions Regarding Varicocele Surgery
Hello Dr.
Hsu,
I had left-sided varicocele surgery last week, and the incision is located on the right side of the left groin (upper abdomen).
The wound is healing well and is not very painful anymore.
However, I have been experiencing a persistent sensation of numbness and swelling at the junction of the left scrotum and abdomen (inguinal area), although it is not painful.
It feels somewhat like a lack of blood circulation leading to numbness.
Lying down and resting helps reduce the swelling, but the numbness persists.
If I sit for a while or experience physiological responses (such as an erection), the swelling worsens.
I consulted the surgeon, who mentioned that some nerve damage post-surgery is a normal phenomenon.
I would like to ask you, Dr.
Hsu, if the numbness and swelling in that area is normal? Additionally, I sometimes experience brief episodes of sharp pain in the testicle; is this also a normal occurrence? Thank you, Dr.
Hsu.

Hello, you should have undergone high ligation for varicose veins.
The nearby neurovascular structures may have been compressed or injured during the surgery.
Local heat application can help increase circulation and may promote faster recovery.
Generally, after high ligation surgery, the swelling of the scrotum may take about six months to fully improve.
If you have any further questions, feel free to ask, and we can discuss and research together.
Please note that online responses should only be used as a reference; it is essential to seek medical attention at a healthcare facility if necessary.

Hello, thank you for your question regarding your post-varicocele surgery experience. It's understandable to have concerns about swelling and numbness after such a procedure, especially when it involves delicate structures like the spermatic cord and surrounding nerves.
Firstly, it's important to recognize that some degree of swelling and numbness can be common after varicocele surgery. The procedure involves manipulating the veins and surrounding tissues, which can lead to temporary changes in sensation and fluid accumulation. The numbness you are experiencing in the left side of your scrotum and the area where it connects to your abdomen may be due to minor nerve irritation or damage that can occur during surgery. This is often a transient condition, and many patients report gradual improvement over time.
The swelling you describe, particularly if it decreases when you lie down, suggests that it may be related to fluid accumulation or post-operative edema. This is a normal response as your body heals. However, if the swelling persists or worsens, or if you notice any signs of infection (such as increased redness, warmth, or discharge from the incision site), it is crucial to follow up with your surgeon.
Regarding the intermittent sharp pain in your testicle, this can also be a normal post-operative symptom. After surgery, the body undergoes a healing process that can involve nerve regeneration, which may lead to sensations of pain or discomfort as the nerves recover. However, if these pains become severe or are accompanied by other symptoms such as swelling or changes in the appearance of the testicle, it would be wise to consult your physician.
In terms of recovery, it is essential to follow your surgeon's post-operative care instructions. This may include avoiding heavy lifting, strenuous activities, or sexual activity for a specified period to allow for proper healing. Gradually resuming normal activities, while listening to your body, is key. If you find that certain positions or activities exacerbate your symptoms, it may be beneficial to modify your movements or take breaks as needed.
Lastly, while some discomfort is expected, persistent or worsening symptoms should not be ignored. If your numbness, swelling, or pain does not improve over the next few weeks, or if you have any concerns about your recovery, do not hesitate to reach out to your healthcare provider for further evaluation. They may recommend imaging studies or additional treatments to ensure that your recovery is on track.
In summary, while some swelling and numbness can be normal after varicocele surgery, it is essential to monitor your symptoms closely. If you have any doubts or if your symptoms change, please consult your surgeon for personalized advice and reassurance. Wishing you a smooth recovery!
