I have been troubled by bloating issues for a long time?

Hello Doctor: Since December, I have been experiencing a cough, cold, and abdominal bloating.
I visited a clinic, and while the cold improved, the abdominal bloating persisted, accompanied by a sensation of excessive gas, frequent burping, and flatulence (a lot).
I also feel like there is something stuck in my throat (intermittently experiencing dry heaving, but no vomiting).
Between January and February, I went to a gastroenterology clinic for examinations.
An abdominal ultrasound showed no abnormalities, and an endoscopy revealed no signs of gastric ulcers, inflammation, or gastroesophageal reflux disease.
Stool tests also returned negative results with no significant issues.
Despite taking gastrointestinal medications for an extended period, my symptoms have not improved, and my overall condition has worsened, affecting my daily life.
The clinic physician suggested a referral.
Could you please advise what these symptoms might indicate and what further detailed examinations and treatment improvements I should pursue? Thank you!

Hello: Based on the symptoms you described, it is initially assessed as dyspepsia, which is essentially a gastrointestinal functional disorder.
You should consider adjusting your dietary habits, such as eating smaller, more frequent meals, chewing thoroughly, avoiding overeating, steering clear of heavily seasoned foods, increasing your fruit intake, drinking warm water, and incorporating regular appropriate exercise.
I believe these changes will improve your symptoms.
Wishing you peace and health.
Sincerely, Dr.
Chen Ruichang, Gastroenterology Department, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Persistent bloating and gas can be quite uncomfortable and disruptive to daily life. Based on the information provided, it seems that you have undergone several diagnostic tests, including abdominal ultrasound, gastroscopy, and stool tests, which have not revealed any significant abnormalities. However, your symptoms persist, and this can be frustrating.

Bloating and excessive gas can arise from various causes, and understanding these can help in managing your symptoms effectively. Here are some potential reasons for your gastrointestinal discomfort:
1. Dietary Factors: Certain foods are known to produce more gas during digestion. Foods high in fermentable carbohydrates, known as FODMAPs (Fermentable Oligosaccharides, Disaccharides, Monosaccharides, and Polyols), can lead to increased gas production. Common culprits include beans, lentils, certain fruits (like apples and pears), dairy products (especially if lactose intolerant), and certain vegetables (like onions and garlic). Keeping a food diary can help identify if specific foods trigger your symptoms.

2. Gut Motility Issues: Conditions such as irritable bowel syndrome (IBS) can lead to symptoms of bloating and gas. IBS is characterized by a combination of abdominal pain, bloating, and changes in bowel habits (diarrhea or constipation). Stress and anxiety can exacerbate these symptoms, so managing stress through relaxation techniques or therapy may be beneficial.

3. Swallowing Air: If you tend to eat quickly, chew gum, or drink carbonated beverages, you may be swallowing excess air, which can contribute to bloating and gas. Being mindful of your eating habits and avoiding these behaviors can help reduce symptoms.

4. Gastroesophageal Reflux Disease (GERD): Although your tests did not show signs of GERD, the sensation of a lump in your throat and dry heaving could suggest some degree of reflux or esophageal sensitivity. This can also lead to bloating and discomfort.

5. Microbiome Imbalance: An imbalance in gut bacteria can lead to digestive issues, including bloating and gas. Probiotics may help restore a healthy balance of gut flora, but it's essential to choose the right strains for your specific symptoms.

6. Functional Dyspepsia: This condition involves chronic indigestion without an identifiable cause. Symptoms can include bloating, discomfort, and nausea. Treatment often involves dietary changes and medications to improve gastric motility.

Given that your symptoms have not improved with standard treatments, it may be worthwhile to consider further evaluations. Here are some suggestions:
- Consult a Gastroenterologist: If you haven't already, seeing a specialist who can provide a more in-depth evaluation of your symptoms may be beneficial. They might suggest additional tests, such as breath tests for lactose intolerance or small intestinal bacterial overgrowth (SIBO), which can cause similar symptoms.

- Dietary Consultation: Working with a registered dietitian who specializes in gastrointestinal health can help you identify potential food triggers and develop a personalized eating plan that minimizes bloating and gas.

- Stress Management: Since stress can significantly impact gastrointestinal function, incorporating stress-reduction techniques such as mindfulness, yoga, or cognitive behavioral therapy may help alleviate your symptoms.

- Medication Review: If you are currently taking medications for gastrointestinal symptoms, discussing their effectiveness and potential alternatives with your healthcare provider may be beneficial.

In summary, persistent bloating and gas can stem from various factors, including dietary choices, gut motility issues, and stress. A comprehensive approach that includes dietary modifications, stress management, and possibly further medical evaluation can help you find relief from your symptoms. Remember to communicate openly with your healthcare provider about your ongoing discomfort to explore all possible avenues for treatment.
