Pearly penile papules
Hello doctor, I apologize for bothering you.
I would like to ask about something I recently noticed.
I found small white bumps in the middle of my foreskin.
At first, I thought they resembled sebaceous glands, but I read online that sebaceous glands are usually located on the posterior side.
There are quite a few of these slightly raised white bumps in the middle of the foreskin.
Could you please tell me what this symptom might be and if it requires treatment?
Niming, 30~39 year old female. Ask Date: 2015/04/27
Dr. Li Xinyi reply Urology
It sounds like pearly penile papules or some benign sebaceous changes.
No treatment is necessary.
However, a visit to a urologist for examination is still recommended for a definitive diagnosis.

The presence of white bumps on the foreskin can be concerning, and it's important to understand the potential causes and appropriate treatments. Based on your description, these white bumps could be a few different things, and while I cannot diagnose without a physical examination, I can provide some insights into what you might be experiencing.
One common cause of white bumps on the foreskin is pearls of Fordyce, which are small, painless, pale bumps that are actually sebaceous glands. They are a normal anatomical variation and are not harmful. These bumps are usually not a cause for concern and do not require treatment unless they cause discomfort or anxiety.
Another possibility is pearly penile papules, which are small, dome-shaped bumps that typically appear around the corona of the glans penis. Like Fordyce spots, these are benign and do not require treatment.
However, if the white bumps are accompanied by other symptoms such as itching, burning, or discharge, it could indicate a fungal infection (like a yeast infection), bacterial infection, or even sexually transmitted infections (STIs). For instance, a yeast infection can lead to white patches and discomfort, while STIs may present with various symptoms, including unusual discharge or sores.
If you notice that the bumps are changing in appearance, becoming painful, or if you experience any other concerning symptoms, it is advisable to seek medical attention. A healthcare provider can perform a physical examination and may conduct tests to determine the exact cause of the bumps.
In terms of treatment, if the bumps are determined to be benign, no treatment may be necessary. However, if an infection is diagnosed, appropriate antifungal or antibacterial medications may be prescribed. Over-the-counter treatments may not be effective for infections, so it’s best to consult with a healthcare professional for a proper diagnosis and treatment plan.
In summary, while white bumps on the foreskin can often be benign, it is essential to monitor them for any changes or accompanying symptoms. If you have any doubts or concerns, especially if these bumps are causing discomfort or if you have a history of STIs, it is always best to consult with a healthcare provider for personalized advice and treatment options.
