White Bumps on the Foreskin: Causes and Concerns - Urology

Hello, doctor.
I would like to ask about something.
Recently, I noticed white spots on my foreskin that resemble chicken skin.
If I were to describe it, it appears in the middle area of the foreskin, with small, raised, chicken skin-like bumps.
They are not painful or itchy, and I don't recall noticing them before; I only discovered them recently while grooming.
Could this be a bacterial infection?

Xiao Kai, 20~29 year old female. Ask Date: 2015/04/27

Dr. Li Xinyi reply Urology

It sounds like pearly penile papules.
No treatment is necessary, but it is still important to see a urologist for a proper diagnosis.

When it comes to the appearance of white bumps on the foreskin, it is important to understand that there are several potential causes, and not all of them are indicative of an infection or a serious health concern. The bumps you describe, resembling "chicken skin," could be a benign condition known as pearly penile papules (PPP), which are small, white or flesh-colored bumps that typically appear around the corona of the glans (the head of the penis). These are normal anatomical variations and are not associated with any disease or infection.
Another possibility is that these bumps could be sebaceous cysts, which are small, non-painful lumps that can occur due to blocked sebaceous glands. They are filled with sebum, a natural oil produced by the skin, and are generally harmless.
In some cases, the bumps could be related to a condition called lichen sclerosus, which is a chronic inflammatory skin condition that can affect the genital area. This condition may cause white patches or bumps and can lead to discomfort or pain, but it typically requires a medical diagnosis and treatment.

It is also important to consider sexually transmitted infections (STIs) such as genital warts, which are caused by the human papillomavirus (HPV). These can appear as small, raised bumps and may require medical evaluation and treatment.

Given that you mentioned these bumps are not painful or itchy, it is less likely that they are due to an acute infection. However, it is crucial to consult a healthcare professional, preferably a dermatologist or urologist, for an accurate diagnosis. They can perform a physical examination and may recommend further tests if necessary.

In terms of treatment, if the bumps are determined to be benign, no treatment may be required. However, if they are found to be caused by an infection or another condition, appropriate treatment options can be discussed. For example, if an STI is diagnosed, antiviral or topical treatments may be prescribed.
In summary, while the appearance of white bumps on the foreskin can be concerning, many potential causes are benign. It is essential to seek medical advice for a proper diagnosis and to alleviate any concerns you may have. Regular check-ups and open communication with your healthcare provider can help ensure your sexual health and overall well-being.

Based on the literal description, it is not possible to confirm the condition without actually seeing the lesion. It is recommended to have a physician evaluate it to determine the next steps. If necessary, surgery or a biopsy may be required.
