Is the anal fistula site not fully healed after surgery?

Hello Dr.
Ke: I underwent surgery for an anal fistula about two months ago.
For the first two weeks post-surgery, I continued with warm sitz baths and applied medication.
After some time, the affected area began to heal, and I started to reduce the frequency of these treatments.
Now, I no longer do sitz baths or apply medication.
However, the area in the groin, although it has shown some healing, has not completely healed after two months.
The skin has not returned to its previous smooth state; upon careful examination, it feels a bit uneven.
Nevertheless, compared to before the surgery, where I experienced pus discharge and could feel a noticeable fistula, and sometimes gas would escape from the affected area, those symptoms have not recurred.
Is this normal, doctor? Thank you.

Ahao, 20~29 year old female. Ask Date: 2015/05/06

Dr. Ke Fangxu reply Surgery

It is normal for fistula surgery to result in more noticeable scar tissue, as the procedure involves destroying the fistula tract to prevent recurrence.
As long as there is no pain, inflammation, or abnormal discharge, it is not a concern.
However, it is unlikely that the skin will return to its original smooth appearance.

After anal fistula surgery, it is not uncommon for patients to experience incomplete healing or prolonged recovery times. The healing process can vary significantly from person to person, and several factors can influence the timeline and quality of healing.
Firstly, it is important to understand that the anal region is a complex area with a rich blood supply and numerous nerve endings. This can lead to a slower healing process compared to other parts of the body. The presence of scar tissue, which can develop after surgery, may also contribute to irregularities in the skin texture, such as the "uneven" surface you described. This is particularly true if the surgical site was extensive or if there was significant tissue damage prior to the surgery.

In your case, it seems that the major symptoms associated with the fistula, such as drainage and discomfort, have resolved, which is a positive sign. The fact that you no longer experience pus drainage or the sensation of gas passing through the fistula indicates that the primary issue has been addressed. However, the ongoing irregularity in the skin texture and the slow healing of the groin area may still warrant attention.

It is essential to maintain good hygiene in the area and to follow any post-operative care instructions provided by your surgeon. If you have stopped sitz baths and topical treatments, consider discussing with your healthcare provider whether resuming these practices might be beneficial for your healing process. Sitz baths can help keep the area clean and may promote healing by increasing blood flow to the region.

If the area remains uneven or if you notice any signs of infection (such as increased redness, swelling, or discharge), it is crucial to consult your surgeon or a healthcare professional. They may recommend further evaluation, which could include imaging studies or a physical examination to assess the healing process and rule out any complications.

In some cases, additional treatments may be necessary to promote healing. This could involve minor procedures to remove any excess scar tissue or to ensure that the fistula has completely closed. Your surgeon may also discuss options for managing any discomfort or irregularities in the skin texture.

In summary, while some degree of incomplete healing can be normal after anal fistula surgery, it is essential to monitor the situation closely. If you have concerns about the healing process or if symptoms persist, do not hesitate to reach out to your healthcare provider for further evaluation and guidance. Your recovery is important, and addressing any lingering issues promptly can help ensure the best possible outcome.
