Blood in the stool after a bowel movement?

After wiping with toilet paper, there is blood on it.
I noticed there is blood at the end of my stool, and it feels like there are fissures around my anus.
Shortly after, I experienced abdominal pain and couldn't have a bowel movement.
When I got up, I saw two or three drops of blood.
Should I see a doctor immediately?

Ting, 10~19 year old female. Ask Date: 2015/06/27

Dr. Lin Wenxu reply Urology

The possible cause may be anal fissure.
It is recommended to drink plenty of water, eat more fruits and vegetables, and exercise regularly.
If you continue to feel very uncomfortable, experience difficulty in bowel movements, or have persistent blood in your stool, please visit a colorectal clinic for examination.

More Info

Experiencing blood after a bowel movement can be concerning, and it's important to understand the potential causes and when to seek medical attention. The presence of blood on toilet paper or in the stool can be attributed to several factors, including hemorrhoids, anal fissures, diverticular disease, or more serious conditions such as gastrointestinal bleeding or colorectal cancer.

1. Hemorrhoids: These are swollen veins in the lower rectum or anus that can cause bleeding during bowel movements. They are often associated with straining, constipation, or prolonged sitting. If the blood is bright red and appears on the surface of the stool or on toilet paper, it is likely from hemorrhoids.

2. Anal Fissures: These are small tears in the lining of the anus, often caused by passing hard stools. They can cause sharp pain during and after bowel movements, along with bleeding. The blood is usually bright red and may be seen on the toilet paper or in the toilet bowl.

3. Diverticular Disease: This condition involves the formation of small pouches in the colon wall, which can become inflamed or infected, leading to bleeding. The blood may be more significant and can be mixed with stool.

4. Gastrointestinal Bleeding: This is a more serious condition that can originate from anywhere in the digestive tract. The blood may appear dark or tarry if it comes from higher up in the gastrointestinal tract.

5. Colorectal Cancer: While less common, blood in the stool can be a symptom of colorectal cancer, especially if accompanied by other symptoms such as changes in bowel habits, unexplained weight loss, or abdominal pain.

Given your symptoms of blood on the toilet paper, a feeling of anal fissure, and abdominal pain, it is advisable to see a doctor promptly. While hemorrhoids and anal fissures are common and often not serious, any new onset of rectal bleeding should be evaluated to rule out more serious conditions.

When to See a Doctor:
- If you notice blood in your stool or on toilet paper, especially if it persists.

- If you experience significant pain during bowel movements.

- If you have accompanying symptoms such as severe abdominal pain, changes in bowel habits, or unexplained weight loss.

- If the bleeding is heavy or you feel faint or lightheaded.

What to Expect at the Doctor's Office:
During your visit, the doctor will likely perform a physical examination, which may include a digital rectal exam. They may also recommend further tests such as a colonoscopy, especially if you are over 50 or have risk factors for colorectal cancer.

Management:
If the bleeding is due to hemorrhoids or anal fissures, treatment may include dietary changes to increase fiber intake, topical treatments, or in some cases, procedures to remove hemorrhoids. For other conditions, treatment will depend on the underlying cause.

In conclusion, while many causes of rectal bleeding are benign, it is crucial to take this symptom seriously and consult a healthcare professional to ensure proper diagnosis and treatment. Your health and peace of mind are important, so do not hesitate to seek medical attention.
