Inability of the glans penis to engorge or enlarge?
I have a question.
Recently, when I have an erection, my penis becomes very hard and enlarges, but the glans does not swell or become engorged; it remains soft like when I am not erect.
This has been happening for several months.
Why is this happening? In the past, the glans would also swell and become engorged during an erection.
Is it possible that the glans is unable to become engorged?

Dr. Fu Shizhang reply Urology
The glans is composed of the corpus spongiosum, which is naturally less firm than the corpora cavernosa of the penis.
Therefore, even though it may be softer during an erection, it should still swell.
It is recommended to continue monitoring the situation.
If there is still no engorgement or enlargement, consider going to the hospital for a cavernosography examination.

More Info
The issue you're experiencing, where the glans (the head of the penis) does not swell or become engorged during an erection, can be concerning and may indicate an underlying medical condition. It's important to understand that the penis consists of erectile tissue that fills with blood during an erection, leading to both the shaft and the glans becoming firm and engorged. If the glans is not swelling as it should, there could be several reasons for this phenomenon.
Possible Causes
1. Vascular Issues: The most common reason for inadequate engorgement of the glans during an erection is related to blood flow. If there are issues with the blood vessels supplying the glans, it may not receive enough blood to swell properly. Conditions such as atherosclerosis (hardening of the arteries) or other vascular diseases can impede blood flow.
2. Nerve Damage: The process of achieving an erection is not solely dependent on blood flow; it also involves nerve signals. If there is any nerve damage or dysfunction, it may affect the ability of the glans to engorge. This could be due to diabetes, pelvic surgery, or other neurological conditions.
3. Hormonal Factors: Testosterone plays a significant role in sexual function. Low testosterone levels can lead to changes in erectile function, including the inability of the glans to swell during an erection.
4. Psychological Factors: Stress, anxiety, and other psychological issues can affect sexual performance and arousal. If you are experiencing anxiety about your sexual health, it could manifest in physical symptoms.
5. Peyronie's Disease: This condition involves the development of fibrous scar tissue inside the penis, which can lead to curvature and may affect how the glans engorges during an erection.
6. Infections or Inflammation: Conditions such as balanitis (inflammation of the glans) or other infections can also affect the glans' ability to engorge properly.
When to Seek Medical Attention
Given that this issue has persisted for several months, it is advisable to consult a healthcare professional, preferably a urologist. They can perform a thorough examination, which may include:
- Physical Examination: To check for any visible abnormalities or signs of infection.
- Blood Tests: To assess hormone levels, including testosterone, and to check for diabetes or other underlying conditions.
- Doppler Ultrasound: This test can evaluate blood flow to the penis and help identify any vascular issues.
- Psychological Evaluation: If psychological factors are suspected, a referral to a mental health professional may be beneficial.
Conclusion
In summary, the inability of the glans to swell during an erection can be attributed to various factors, including vascular issues, nerve damage, hormonal imbalances, psychological factors, or infections. It is crucial to seek medical advice to determine the underlying cause and receive appropriate treatment. Early intervention can often lead to better outcomes and help alleviate any concerns you may have about your sexual health. Remember, sexual health is an important aspect of overall well-being, and addressing these issues can lead to improved quality of life.
