There is a popping sound in the ear?

I have a clicking sound in my ear, but I can control when I hear it and when I don't (it's been like this for several years).
Recently, it has suddenly changed to making a clicking sound when I talk, and I can no longer control it.
There is no pain involved.
I'm not sure what is happening...

A San, 10~19 year old female. Ask Date: 2015/07/06

Dr. Zheng Jueyi reply Otolaryngology

Hello, A-San: It is possible that small earwax is lodged near the eardrum or that the sound is caused by the temporomandibular joint.
If it does not improve after a week, it is advisable to seek medical evaluation for a diagnosis.
Wishing you safety and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

The clicking sounds you are experiencing in your ears, particularly when speaking or making certain movements, can be attributed to several potential causes, most commonly related to the Eustachian tube, the structure that connects the middle ear to the back of the nose. This tube plays a crucial role in equalizing pressure in the middle ear and draining fluid. When it does not function properly, it can lead to a sensation of fullness or clicking sounds.

Possible Causes of Clicking Sounds in the Ears
1. Eustachian Tube Dysfunction (ETD): This is one of the most common reasons for clicking sounds in the ears. ETD occurs when the Eustachian tube becomes blocked or fails to open properly. This can happen due to allergies, sinus infections, or even changes in altitude. When you speak or swallow, the pressure changes can cause the tube to open and close irregularly, resulting in a clicking sound.

2. Middle Ear Pressure Changes: If there are fluctuations in pressure within the middle ear, such as during altitude changes or due to nasal congestion, you may hear clicking sounds. This is often accompanied by a feeling of fullness or pressure in the ear.

3. Muscle Spasms: Sometimes, the muscles around the Eustachian tube can spasm, leading to clicking or popping sounds. This can occur during certain movements or when speaking loudly.

4. Earwax Buildup: Excessive earwax can also cause sounds in the ear. If the earwax is pressing against the eardrum or moving with jaw movements, it can create a clicking noise.

5. Temporomandibular Joint (TMJ) Disorders: Issues with the jaw joint can also lead to sounds in the ear. The TMJ is located very close to the ear, and problems in this area can cause referred sounds or sensations.

When to Seek Medical Attention
While clicking sounds in the ears can often be benign, especially if they are not accompanied by pain, hearing loss, or other concerning symptoms, it is advisable to consult with an ear, nose, and throat (ENT) specialist if:
- The clicking sounds persist or worsen over time.

- You experience any hearing loss or changes in your hearing.

- There are accompanying symptoms such as dizziness, pain, or a feeling of fullness that does not resolve.

- You have a history of ear infections or other ear-related issues.

Diagnostic Approaches
An ENT specialist may perform a thorough examination, which could include:
- Audiometry: To assess your hearing ability.

- Tympanometry: To evaluate the function of the middle ear and Eustachian tube.

- Nasal Endoscopy: To check for any obstructions or issues in the nasal passages that could be affecting the Eustachian tube.

Treatment Options
Treatment will depend on the underlying cause of the clicking sounds. Options may include:
- Decongestants or Antihistamines: If allergies or nasal congestion are contributing to Eustachian tube dysfunction.

- Earwax Removal: If excessive earwax is identified as the cause.

- Eustachian Tube Exercises: Techniques such as swallowing, yawning, or the Valsalva maneuver can help equalize pressure in the ears.

- TMJ Treatment: If TMJ disorders are suspected, treatments may include physical therapy, dental splints, or other interventions.

In conclusion, while clicking sounds in the ears can be concerning, they are often manageable with appropriate medical guidance. If your symptoms persist or change, seeking an evaluation from an ENT specialist is the best course of action to ensure proper diagnosis and treatment.
