There is a sound in the ear that resembles a pen cap?

Hello, I often hear a sound like "enn enn enn" in my ear, sometimes accompanied by a clicking sound similar to pressing a pen cap, which is quite clear.
It seems to occur only in my left ear, and when the clicking sound happens, I feel a sensation as if there is a pen inside my ear.
However, there are no foreign objects in my ear, and the sounds tend to occur more frequently at night.
There have been no other abnormalities, and this has been going on for about a year.
I'm not sure what it could be, so I would appreciate any feedback from the doctor.
Thank you.

Hello, if the tinnitus occurs only during quiet times, it is mostly a benign phenomenon.
You can observe it for now, and if it does not improve, you may go to the hospital for relevant examinations.
Wishing you peace and good health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

More Info

Understanding ear noises, particularly clicking or popping sounds, can be perplexing and concerning for many individuals. Based on the symptoms you've described, it seems that you are experiencing a phenomenon often referred to as "ear noises" or "tinnitus." Tinnitus can manifest in various forms, including ringing, buzzing, clicking, or even a sensation of fullness in the ear.

Possible Causes of Ear Noises
1. Eustachian Tube Dysfunction: One of the most common causes of clicking sounds in the ear is Eustachian tube dysfunction. The Eustachian tube connects the middle ear to the back of the throat and helps equalize pressure. When this tube does not open properly, it can create a sensation of pressure and lead to clicking sounds, especially during activities that change ear pressure, such as swallowing or yawning.

2. Earwax Buildup: Another potential cause could be the accumulation of earwax (cerumen) in the ear canal. While you mentioned that there are no foreign objects in your ear, sometimes even small amounts of earwax can cause sounds when it shifts or moves, particularly when you change positions or apply pressure to the ear.

3. Muscle Spasms: The muscles in and around the ear can sometimes spasm, leading to sounds that may feel like clicking or popping. This can occur due to stress, fatigue, or even certain movements of the jaw.

4. Tinnitus: As mentioned earlier, tinnitus is a condition characterized by hearing sounds that are not present in the environment. It can be caused by a variety of factors, including exposure to loud noises, ear infections, or even stress and anxiety. The fact that your symptoms are more pronounced at night could suggest that they are related to stress or fatigue.

5. Changes in Air Pressure: Activities that involve changes in air pressure, such as flying or diving, can also lead to sensations in the ear. However, since you mentioned that these sounds occur primarily at night, this may not be the primary cause in your case.

Recommendations for Management
1. Consult an Ear, Nose, and Throat Specialist: Since your symptoms have persisted for about a year, it would be wise to consult with an ENT specialist. They can perform a thorough examination, including checking for earwax buildup, assessing the Eustachian tube function, and evaluating your overall ear health.

2. Avoid Irritants: If you suspect that stress or anxiety may be contributing to your symptoms, consider engaging in relaxation techniques such as deep breathing, meditation, or yoga. Reducing caffeine and alcohol intake may also help alleviate symptoms.

3. Ear Hygiene: Maintain good ear hygiene, but avoid inserting objects into your ear canal, as this can push earwax further in and potentially cause blockages.

4. Sound Therapy: If tinnitus is diagnosed, sound therapy or white noise machines can help mask the sounds and make them less noticeable, especially at night.

5. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their intensity, and any associated activities. This information can be valuable for your healthcare provider in determining the underlying cause.

Conclusion
In summary, the clicking sounds in your ear could be attributed to various factors, including Eustachian tube dysfunction, earwax buildup, muscle spasms, or tinnitus. Given the duration and nature of your symptoms, seeking professional medical advice is crucial for an accurate diagnosis and appropriate management. Remember, while ear noises can be bothersome, they are often manageable with the right approach.
