Head trauma

Hello doctor, on Tuesday at noon, I bumped the top of my head against the door frame of a children's restroom.
At the moment of impact, I did not lose consciousness, just felt a bit dazed.
I can walk normally, but there is a persistent sharp pain at the site of the impact, along with frequent feelings of nausea, blurred vision, and mild dizziness.
Should I see a doctor for this?

Dr. Jiang Junyi reply Neurology

Hello, based on your description, generally speaking, if there is no loss of consciousness, the use of anticoagulants, or obvious local hematomas, head injuries are mostly minor (such as localized pain).
However, if the following conditions occur, delayed bleeding should be considered, and you should seek emergency care: (1) Deterioration of vision, seeing double (diplopia), or unequal pupil size.
(2) Watery fluid or blood leaking from the ears or nose.
(3) Confusion or abnormal behavior.
(4) Numbness or inability to move limbs.
(5) Seizures.
Wishing you good health and peace.

When it comes to head injuries, it is crucial to understand the signs and symptoms that warrant immediate medical attention. Based on your description, you experienced a bump on your head from a door frame, which resulted in persistent pain, nausea, visual disturbances, and mild dizziness. These symptoms can be indicative of a concussion or other forms of traumatic brain injury (TBI), even if you did not lose consciousness at the time of the injury.

In general, the following symptoms after a head injury should prompt you to seek medical attention:
1. Loss of Consciousness: Even a brief loss of consciousness can indicate a more serious injury.

2. Persistent or Worsening Headache: If the headache does not improve or worsens over time, it is a sign that you should see a doctor.

3. Nausea or Vomiting: Feeling nauseous or vomiting after a head injury can be a sign of increased intracranial pressure or a concussion.

4. Dizziness or Balance Issues: Difficulty walking or maintaining balance can indicate a problem with the brain's function.

5. Visual Disturbances: Blurred vision, double vision, or other visual changes can be concerning.

6. Confusion or Memory Problems: Feeling dazed or having trouble remembering events surrounding the injury is a red flag.

7. Seizures: Any seizure activity following a head injury is an emergency.

8. Fluid or Blood from Nose or Ears: This can indicate a skull fracture and requires immediate medical evaluation.

Given that you are experiencing nausea, visual disturbances, and dizziness, it is advisable to seek medical attention. A healthcare professional may perform a neurological examination and possibly imaging studies such as a CT scan to rule out any serious injuries like a concussion, contusion, or intracranial hemorrhage.

In addition to seeking medical care, here are some general recommendations for managing minor head injuries:
- Rest: Allow your brain to recover by getting plenty of rest. Avoid strenuous activities and cognitive tasks that require concentration.

- Monitor Symptoms: Keep track of your symptoms. If they worsen or new symptoms develop, seek medical attention promptly.

- Avoid Alcohol and Drugs: These substances can impair your judgment and may exacerbate symptoms.

- Pain Management: Over-the-counter pain relievers like acetaminophen can help manage headaches, but avoid NSAIDs like ibuprofen or aspirin until cleared by a doctor, as they can increase bleeding risk.

In conclusion, while minor bumps to the head may not always result in serious injury, the symptoms you are experiencing suggest that it would be prudent to consult a healthcare professional for a thorough evaluation. Early intervention can help prevent complications and ensure a safe recovery.
