Pustules on the penis?

Hello, I would like to ask about a wound on the middle of my penis that produces pus when squeezed.
I'm very worried it might be syphilis.
The wound looks like a pimple, is not red, but when squeezed, it releases yellow pus without any blood.
I have engaged in oral sex only.
I discovered the wound while showering, and it has been four days now without improvement.
If it is syphilis, can it develop that quickly? The wound could have been caused by the sexual activity, but it wasn't very intense.

Peter, 20~29 year old female. Ask Date: 2015/07/20

Dr. Xu Weikai reply Urology

Peter: Hello, syphilis can usually be diagnosed through a blood test.
It is recommended to have a blood test three months later.

Hello! Thank you for reaching out with your concerns regarding the pus-filled lesion on your penis. It’s understandable to feel anxious about any unusual symptoms, especially in such a sensitive area. Let’s break down your situation and address your concerns.

From your description, it sounds like you have a lesion that produces pus when squeezed, which can be indicative of a few different conditions. The fact that it resembles a pimple and is not red suggests it may not be an acute infection, but it’s important to consider several possibilities.

1. Folliculitis: This is an inflammation of the hair follicles, which can occur anywhere on the body, including the genital area. It can present as small, pus-filled bumps that may resemble pimples. Folliculitis can be caused by bacteria, irritation, or even fungal infections.

2. Pearly Penile Papules: These are small, harmless bumps that can appear around the corona of the penis. They are normal anatomical variations and are not a cause for concern. However, they do not typically produce pus.

3. Sexually Transmitted Infections (STIs): While you mentioned that you have only engaged in oral sex, it’s still possible to contract certain STIs, including herpes or syphilis, which can present with lesions. However, syphilis typically presents with a painless sore (chancre), and herpes usually causes painful blisters. If you are concerned about STIs, it’s crucial to get tested.

4. Abscess: If the lesion is indeed an abscess, it may require drainage and antibiotic treatment. Abscesses can form when bacteria infect a hair follicle or skin, leading to pus accumulation.

Given that the lesion has not improved after four days, it’s advisable to seek medical attention. A healthcare provider can perform a physical examination and may take a swab of the pus for laboratory analysis to determine the exact cause of the lesion. They may also ask about your sexual history and any other symptoms you might be experiencing.

Regarding your concern about syphilis, it’s important to note that while some STIs can develop quickly, others may take longer to manifest symptoms. Testing is the only way to confirm or rule out an STI. If you are experiencing any additional symptoms, such as fever, swollen lymph nodes, or systemic symptoms, you should seek medical care promptly.

In the meantime, avoid squeezing or manipulating the lesion further, as this can exacerbate irritation or lead to secondary infections. Keep the area clean and dry, and consider wearing loose-fitting underwear to minimize friction.

In summary, while there are several potential causes for the pus-filled lesion on your penis, it’s essential to consult a healthcare professional for an accurate diagnosis and appropriate treatment. Early intervention can help prevent complications and provide peace of mind. Don’t hesitate to reach out to a doctor or a sexual health clinic for further evaluation. Your health and well-being are paramount!
