Blood-streaked nasal discharge?

Hello, doctor: For the past three days, I have felt discomfort in my left nostril upon waking up.
It feels like there is thick mucus that is difficult to expel.
After sneezing, I can clear out a small amount, which resembles brown phlegm.
The discomfort in my throat feels somewhat like tonsillitis.
The symptoms do not last long, about an hour before they subside.
I have a history of allergic rhinitis and would like to know if this is related to nasopharyngeal cancer.

Qiu Xiaojie, 20~29 year old female. Ask Date: 2015/07/23

Dr. Zheng Jueyi reply Otolaryngology

Hello: If there is a history of allergic rhinitis, such symptoms are often related to sinusitis.
If symptoms do not improve after one week, it is recommended to visit an otolaryngologist to confirm whether there is an issue with the sinuses.
Wishing you safety and health.
Sincerely, Dr.
Cheng Jue-Yi, Director of the Department of Otolaryngology, Taoyuan Hospital, Ministry of Health and Welfare.

Understanding blood-streaked nasal discharge can be concerning, especially when accompanied by other symptoms. Based on your description, it seems you are experiencing a combination of nasal discomfort, colored nasal discharge, and throat irritation. Let’s break down the potential causes and concerns regarding your symptoms.

Possible Causes of Blood-Streaked Nasal Discharge
1. Nasal Irritation and Trauma: One of the most common causes of blood-streaked nasal discharge is irritation or trauma to the nasal mucosa. This can occur from frequent nose blowing, sneezing, or even dry air, which can lead to the rupture of small blood vessels in the nasal lining. Given your history of allergic rhinitis, it is possible that your nasal passages are inflamed, making them more susceptible to bleeding.

2. Infection: Conditions such as sinusitis or upper respiratory infections can lead to thickened nasal mucus, which may appear discolored (brown or yellow) due to the presence of pus or blood. The sensation of throat discomfort could also indicate post-nasal drip, where mucus drips down the back of the throat, causing irritation.

3. Allergic Rhinitis: Since you have a history of allergic rhinitis, it’s important to consider that allergies can cause inflammation and swelling in the nasal passages. This inflammation can lead to increased mucus production and potential bleeding, especially if you are frequently blowing your nose or if the nasal passages are dry.

4. Nasal Polyps or Other Growths: Although less common, nasal polyps or other growths in the nasal cavity can cause obstruction and lead to irritation and bleeding. However, these are usually associated with chronic symptoms rather than acute episodes.

5. Serious Conditions: While it is less likely, persistent blood-streaked nasal discharge can sometimes be associated with more serious conditions, such as nasal tumors (including nasopharyngeal carcinoma). However, these typically present with additional symptoms such as persistent nasal obstruction, facial pain, or swelling, and are less common in younger individuals without significant risk factors.

Concerns Regarding Nasopharyngeal Cancer
Your concern about nasopharyngeal cancer is understandable, especially with the presence of blood in the nasal discharge. However, it is important to note that nasopharyngeal cancer typically presents with more pronounced symptoms, including:
- Persistent nasal obstruction
- Unexplained weight loss
- Ear pain or fullness
- Swollen lymph nodes in the neck
- Persistent sore throat or difficulty swallowing
If you do not have these additional symptoms, it is less likely that your current condition is related to cancer. However, if you have a family history of cancer or other risk factors, it is advisable to discuss these concerns with your healthcare provider.

Recommendations
1. Monitor Symptoms: Keep track of your symptoms, including the frequency and duration of the blood-streaked discharge, as well as any additional symptoms that may arise.

2. Avoid Irritants: Try to avoid irritants such as smoke, strong odors, and allergens that may exacerbate your symptoms.

3. Hydration and Humidification: Staying well-hydrated and using a humidifier can help keep your nasal passages moist and reduce irritation.

4. Consult a Specialist: If your symptoms persist or worsen, it is advisable to consult an ear, nose, and throat (ENT) specialist. They may perform a nasal endoscopy to examine your nasal passages and determine the underlying cause of your symptoms.

5. Allergy Management: If you have not already, consider discussing allergy management strategies with your healthcare provider, which may include antihistamines or nasal corticosteroids to reduce inflammation.

In conclusion, while blood-streaked nasal discharge can be alarming, it is often related to benign causes such as nasal irritation or infection, especially in the context of allergic rhinitis. However, persistent or concerning symptoms warrant further evaluation by a healthcare professional to rule out more serious conditions.
