Abnormal menstrual bleeding

Doctor, I would like to ask about my experience after receiving a menstrual induction injection.
My period started a week later and lasted for five days, during which the flow decreased to the point where I only needed a panty liner by the seventh day.
On the eighth day, it was almost gone.
However, after having sexual intercourse, I went to the bathroom less than an hour later and noticed some blood clots along with a moderate amount of menstrual blood—not too much, but not too little either.
After that, the bleeding turned into just a little bit of bright red color.
This is my first time receiving a menstrual induction injection; I have previously only taken menstrual induction pills.
Is this situation considered normal?

An, 20~29 year old female. Ask Date: 2015/08/09

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, based on your description, it seems that there may have been an underlying hormonal imbalance, which could explain the irregular menstrual cycle and the subsequent irregular bleeding between periods.
It is advisable to consult a gynecologist if the irregularities in your menstrual cycle or abnormal bleeding persist.
Thank you! Wishing you peace and health, Dr.
Chang.

Abnormal menstrual bleeding after hormonal injections, such as those used to induce menstruation, can be concerning and may lead to questions about what is considered normal. In your case, it seems that you received an injection to induce your period, and you experienced a relatively typical pattern of bleeding initially, which then changed in volume and consistency over the course of several days.

When hormonal injections are administered, they can cause various changes in the menstrual cycle. The initial bleeding you experienced after the injection is likely a withdrawal bleed, which is a response to the sudden change in hormone levels. This is similar to what happens when women take hormonal contraceptives and then stop them, leading to a withdrawal bleed. The fact that your bleeding decreased significantly after the fifth day and then changed to spotting is not uncommon.
The presence of blood clots and changes in the color of the blood can also be attributed to the hormonal fluctuations caused by the injection. Blood clots can occur during menstruation, especially when the flow is heavier, and they are generally not a cause for concern unless they are very large or accompanied by severe pain. The bright red color of the blood you observed after sexual intercourse could indicate fresh bleeding, which can happen due to various reasons, including cervical irritation or changes in the uterine lining.

It's important to note that every woman's body responds differently to hormonal treatments, and variations in menstrual bleeding patterns can occur. Factors such as stress, changes in weight, and underlying health conditions can also influence menstrual cycles. If you have previously taken hormonal pills and are now using injections for the first time, your body may still be adjusting to this new method of hormone administration.

However, if you notice any of the following symptoms, it would be advisable to consult a healthcare professional:
1. Severe Pain: If you experience significant abdominal or pelvic pain, it could indicate a more serious issue.

2. Heavy Bleeding: Soaking through a pad or tampon every hour for several consecutive hours may require medical attention.

3. Prolonged Bleeding: If your bleeding continues for more than a week or if you have irregular cycles after this injection, it’s worth discussing with your doctor.

4. Foul Odor: If you notice a foul smell accompanying the bleeding, it could indicate an infection.

In summary, while your symptoms may be within the range of normal responses to hormonal injections, it is essential to monitor your condition. If you have any concerns or if your symptoms change, seeking medical advice is always a prudent course of action. Your healthcare provider can offer personalized guidance based on your medical history and current situation.
