Could you please provide more details about the symptoms?

I would like to ask the doctor a few questions.

1.
I have experienced occasional shortness of breath and difficulty breathing since middle school.
Sometimes these symptoms disappear for a while, but recently they have been occurring frequently.

2.
I am 175 cm tall and weigh around 60 kg.
I was about 63 kg recently, but I suddenly lost weight down to around 58 kg.
My appetite is normal, and no matter how much I eat, I don't gain weight.
Could this be hyperthyroidism?
3.
I would like to ask about the symptoms of diabetes and hypertension.
For diabetes, symptoms like increased thirst and urination are present, but I have not noticed any significant changes in my routine.
Are there more definitive symptoms? Recently, I am worried that I might have diabetes, which could lead to erectile dysfunction.
I have consulted both traditional and Western medicine, and my hormone levels were normal.
However, I did not undergo any specific tests.
When I asked some questions, I received vague answers, which left me confused.
They only mentioned prescribing Viagra for erectile dysfunction next time.
Therefore, I am considering other avenues for examination.
I would like to know the key symptoms of diabetes and hypertension!

Niming, 20~29 year old female. Ask Date: 2015/08/19

Dr. Chen Qingbin reply Internal Medicine

Hello: It is recommended that you visit a hospital for further examination of hyperthyroidism and other related conditions, which require blood tests.
Additionally, issues related to sexual dysfunction should be evaluated by a urologist for proper diagnosis.
Thank you.

Understanding your symptoms is crucial, especially when it comes to issues like breathing difficulties, weight loss, and concerns about diabetes and hypertension. Let's break down your concerns systematically.

Breathing Issues
You mentioned experiencing episodes of difficulty breathing since middle school, with recent occurrences becoming more frequent. Breathing difficulties can stem from various causes, including anxiety, asthma, allergies, or even structural issues in the respiratory system. Given that these episodes have been intermittent, it may be beneficial to consider whether they are triggered by specific situations or environmental factors, such as allergens or stress.
If you find that your breathing issues are accompanied by other symptoms like chest tightness, wheezing, or a feeling of suffocation, it could indicate a more serious condition such as asthma or even panic attacks. It’s essential to consult a healthcare provider for a thorough evaluation, which may include pulmonary function tests or allergy testing.

Weight Loss
You noted a significant drop in weight from 63 kg to 58 kg, despite maintaining a normal appetite. Unintentional weight loss can be concerning and may indicate underlying health issues. One possibility you mentioned is hyperthyroidism, which can lead to increased metabolism, weight loss, and symptoms such as anxiety, tremors, and heat intolerance. A simple blood test measuring thyroid hormone levels (TSH, T3, and T4) can help determine if this is the case.

Other potential causes of unexplained weight loss include gastrointestinal disorders, chronic infections, or even malignancies. It’s crucial to discuss this symptom with your healthcare provider, who may recommend further testing based on your overall health and medical history.

Diabetes Concerns
You expressed worry about diabetes, particularly regarding symptoms like increased thirst and urination. Classic symptoms of diabetes include excessive thirst (polydipsia), frequent urination (polyuria), increased hunger (polyphagia), fatigue, and blurred vision. However, it’s important to note that not everyone with diabetes will experience all these symptoms, especially in the early stages.

To assess your risk for diabetes, your doctor may recommend blood tests such as fasting blood glucose, HbA1c, or an oral glucose tolerance test. These tests will provide a clearer picture of your blood sugar levels and whether they fall within the normal range or indicate prediabetes or diabetes.

Hypertension
Regarding hypertension, it is often referred to as a "silent killer" because it may not present noticeable symptoms until significant damage has occurred. Common indicators can include headaches, shortness of breath, or nosebleeds, but many individuals remain asymptomatic. Regular monitoring of blood pressure is essential, especially if you have risk factors such as a family history of hypertension, obesity, or a sedentary lifestyle.

Conclusion
Given the complexity of your symptoms—breathing difficulties, weight loss, and concerns about diabetes and hypertension—it is crucial to seek a comprehensive evaluation from a healthcare professional. They can conduct the necessary tests, provide a diagnosis, and develop a tailored treatment plan.
In the meantime, consider keeping a symptom diary to track when your breathing issues occur, any associated triggers, and changes in your weight or appetite. This information can be invaluable for your healthcare provider in understanding your condition better.

Remember, early intervention can lead to better outcomes, so don’t hesitate to reach out for help. Your health is paramount, and addressing these concerns proactively is the best approach.
