Red spots on the glans?
Hello Doctor, since August of last year, I have noticed one or two red spots on my glans.
They are not painful or itchy, and there is no discomfort or discharge; they tend to disappear on their own, but sometimes new ones appear in different locations.
This cycle has continued, and I still have this condition.
I visited a doctor once, who diagnosed me with balanitis and prescribed a topical medication, which helped the spots disappear quickly, but they keep recurring.
I feel quite distressed.
Recently, after being intimate with my girlfriend, the spots that were almost gone suddenly became very red, and I even noticed new red spots.
There are small patches of redness on the inner surface of the foreskin.
I always use a condom during intimacy and take care of my hygiene, gently washing with cold water.
I would like to ask the doctor if I am sick.
I apologize for the lengthy message, and thank you for your patience in reading it.
Thank you.

Dr. Xu Weikai reply Urology
A-Jian: Hello, there are many reasons that can cause balanitis, such as allergies to condoms, excessive cleaning of the glans leading to skin damage, etc.
For cleaning the glans, it is generally recommended to use only cool water and to avoid any soaps, body washes, or other cleaning products.
After washing, you can apply petroleum jelly (available at drugstores) locally.
If you have any concerns, you can visit a nearby medical facility for relevant tests or further treatment.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

More Info
The presence of red spots on the glans (the head of the penis) can be concerning, especially when they appear intermittently and are associated with other symptoms or changes in appearance. Based on your description, it seems that you have been experiencing these red spots since August of last year, and they tend to appear and disappear without causing pain, itching, or discharge. You also mentioned that after intimate contact, the spots can become more pronounced, and you have experienced some redness on the inner foreskin as well.
There are several potential causes for red spots on the glans, and understanding these can help in addressing your concerns:
1. Irritation or Inflammation: Conditions such as balanitis (inflammation of the glans) can cause redness and irritation. This can be due to various factors, including poor hygiene, sensitivity to soaps or detergents, or friction during sexual activity. The fact that you have been prescribed a topical treatment (likely a corticosteroid or antifungal) suggests that inflammation was suspected.
2. Allergic Reactions: Sometimes, contact dermatitis can occur due to an allergic reaction to substances such as latex (from condoms), lubricants, or personal hygiene products. If you notice that the red spots worsen after using certain products, this could be a contributing factor.
3. Infections: While you mentioned that there is no pain or discharge, it is still possible that a mild infection could be present. Fungal infections, such as candidiasis, can cause red spots and are often exacerbated by moisture and warmth. Bacterial infections could also be a concern, especially if there is any irritation or injury to the skin.
4. Pearly Penile Papules: These are small, harmless bumps that can appear around the glans and are often mistaken for something more serious. They are normal anatomical variations and do not require treatment.
5. Sexually Transmitted Infections (STIs): Although you are using condoms and practicing good hygiene, it is still important to consider the possibility of STIs, especially if you have had multiple partners. Some STIs can present with mild symptoms or be asymptomatic.
Given that the red spots have been recurring and can become more pronounced after intimate contact, it would be advisable to consult a healthcare provider for a thorough examination. They may recommend:
- A Physical Examination: A healthcare professional can assess the spots and determine if they are indicative of a specific condition.
- Laboratory Tests: If an infection is suspected, tests may be conducted to identify any underlying causes, including STIs.
- Review of Hygiene Practices: Discussing your hygiene routine and any products you use can help identify potential irritants or allergens.
In the meantime, here are some general recommendations:
- Maintain Good Hygiene: Continue to keep the area clean, but avoid harsh soaps or irritants. Gentle washing with water is often sufficient.
- Avoid Irritants: If you suspect that certain products may be causing irritation, consider switching to hypoallergenic options.
- Monitor Symptoms: Keep track of any changes in the appearance of the spots, as well as any new symptoms that may arise.
In conclusion, while the red spots you are experiencing may not be indicative of a serious condition, it is essential to seek medical advice for a proper diagnosis and treatment plan. Your healthcare provider can provide reassurance and help you manage any underlying issues effectively.
