Hemorrhoids during pregnancy
Hello, doctor.
I am currently five months pregnant and have noticed a protrusion near my anus.
I consulted a surgeon who diagnosed it as hemorrhoids and pushed it back in, advising me to take warm sitz baths and apply ointment.
However, despite this, I still experience pain and swelling, and when I touch it, the skin around the protrusion hurts.
There is no discharge or bleeding, but I have been dealing with this for almost a week now, and the warm sitz baths and ointment don't seem to be effective.
What should I do?

Dr. Ke Fangxu reply Surgery
This condition is likely a thrombosed hemorrhoid, which can also occur during pregnancy due to poor blood circulation leading to swelling of the hemorrhoids.
Surgical treatment is not recommended at this stage, so the use of topical medications and warm sitz baths are advised.
It is essential to avoid constipation and prolonged straining during bowel movements during this period, as these can hinder symptom improvement.
If a definitive solution for the hemorrhoid issue is desired, it is generally recommended to address it after delivery.

More Info
Managing hemorrhoids during pregnancy can be quite challenging, especially as the body undergoes significant changes that can exacerbate this condition. Hemorrhoids are swollen veins in the lower rectum and anus, and they can cause discomfort, pain, and bleeding. Given that you are five months pregnant and experiencing these symptoms, it is essential to address them effectively to ensure your comfort and well-being.
Understanding Hemorrhoids in Pregnancy
During pregnancy, hormonal changes and the increased pressure from the growing uterus can lead to the development of hemorrhoids. The increased blood volume and changes in blood flow can also contribute to swelling in the rectal area. Symptoms may include pain, itching, and discomfort, especially when sitting or during bowel movements. It is not uncommon for hemorrhoids to protrude outside the anus, which can be particularly uncomfortable.
Effective Relief Strategies
1. Warm Sitz Baths: While you mentioned that warm sitz baths have not provided much relief, they are still one of the most recommended treatments. Soaking in warm water for 10-15 minutes several times a day can help reduce swelling and soothe the area. Ensure the water is not too hot, as this can cause additional irritation.
2. Topical Treatments: Over-the-counter creams and ointments specifically designed for hemorrhoids can provide temporary relief from pain and itching. Look for products containing witch hazel or hydrocortisone. However, always consult your healthcare provider before using any new medication during pregnancy.
3. Dietary Adjustments: A high-fiber diet is crucial in managing hemorrhoids. Incorporate plenty of fruits, vegetables, whole grains, and legumes into your meals to help soften stools and reduce straining during bowel movements. Staying well-hydrated by drinking plenty of water is also essential.
4. Stool Softeners: If dietary changes are not enough, consider discussing the use of stool softeners with your healthcare provider. These can help ease bowel movements and reduce the strain that can worsen hemorrhoids.
5. Avoid Straining: When using the bathroom, try to avoid straining or sitting for prolonged periods. If you feel the urge to go, do so promptly to prevent constipation and further irritation.
6. Exercise: Gentle exercise, such as walking or prenatal yoga, can improve circulation and help prevent constipation. However, avoid heavy lifting or strenuous activities that may exacerbate your symptoms.
7. Cold Compresses: Applying a cold compress to the affected area can help reduce swelling and numb the pain. Just be sure not to apply ice directly to the skin; wrap it in a cloth first.
8. Consult Your Doctor: Since your symptoms have persisted for a week without significant improvement, it may be beneficial to follow up with your healthcare provider. They can assess the severity of your hemorrhoids and may recommend additional treatments, such as prescription medications or, in some cases, procedures to remove or reduce the hemorrhoids.
When to Seek Immediate Help
If you experience severe pain, significant bleeding, or if the hemorrhoids become thrombosed (a clot forms inside the hemorrhoid), seek medical attention promptly. These conditions may require more intensive treatment.
Conclusion
Managing hemorrhoids during pregnancy requires a combination of lifestyle changes, dietary adjustments, and possibly medical treatments. While it can be uncomfortable, many women experience relief with the right strategies. Always keep an open line of communication with your healthcare provider to ensure that you are receiving the best care tailored to your specific needs during this important time. Remember, your comfort and health are paramount, and addressing these symptoms early can lead to a more pleasant pregnancy experience.
