Which department should I see for my condition?

Hello Dr.
Tseng: I have been experiencing frequent episodes of overheating, which I am already very sensitive to.
When it gets severe, I have intense headaches and excessive sweating.
I have also been dealing with anxiety for two years, and recently, I went to the emergency room once in the last two weeks for a sedative.
Additionally, I often experience leg cramps and weakness at night, but the cause remains undetermined.
Furthermore, I have bone spurs in my hands and occasional weakness.
Since I am currently seeing specialists in cardiology, pulmonology, gastroenterology, traditional Chinese medicine, and have visited the emergency department, I would like to know which specialist would be most appropriate for my symptoms: neurosurgery, neurology, psychiatry, or internal medicine? Why can't I seem to find the cause?

Hello, user: I recommend that you consult a neurologist for autonomic nervous system testing and thyroid endocrine system evaluation.
Thank you for your question.

Hello! It sounds like you are experiencing a complex array of symptoms that can be quite distressing. Given your situation, it’s important to approach this methodically to ensure that you receive the appropriate care.

Based on the symptoms you've described—intense heat sensitivity, severe headaches, excessive sweating, anxiety, leg cramps, and issues with your hands (such as bone spurs and weakness)—it would be prudent to consider a few different specialties.

1. Neurology: Since you are experiencing headaches, leg cramps, and weakness in your hands, a neurologist would be a good option. Neurologists specialize in disorders of the nervous system, which includes the brain, spinal cord, and nerves. They can help determine if there are any underlying neurological issues contributing to your symptoms.

2. Psychiatry: Given your history of anxiety disorder, it may also be beneficial to consult a psychiatrist. Anxiety can manifest in various physical symptoms, including sweating, headaches, and muscle tension. A psychiatrist can evaluate whether your anxiety is exacerbating your physical symptoms and can provide appropriate treatment options, which may include therapy or medication.

3. Internal Medicine: An internist can provide a comprehensive evaluation of your overall health. They can help rule out any systemic issues that might be causing your symptoms, such as hormonal imbalances, metabolic disorders, or other underlying medical conditions.

4. Endocrinology: If your symptoms include significant heat intolerance and sweating, it might be worth consulting an endocrinologist. They specialize in hormonal and metabolic disorders, which could be relevant if you have issues related to thyroid function or other hormonal imbalances.

5. Rheumatology: If you suspect that your bone spurs and joint issues are part of a broader systemic condition, a rheumatologist could be helpful. They specialize in autoimmune and inflammatory conditions that can affect the joints and muscles.

6. Pain Management: If your symptoms include chronic pain or discomfort, a pain management specialist could provide strategies to help manage your symptoms effectively.

Given that you have already seen multiple specialists without a clear diagnosis, it may be beneficial to keep a detailed symptom diary. Document when your symptoms occur, their severity, any potential triggers, and how long they last. This information can be invaluable for any healthcare provider you consult.

In conclusion, I recommend starting with a neurologist for your headaches and neurological symptoms, while also considering a psychiatrist for your anxiety. An internist can help coordinate your care and rule out any systemic issues. It’s essential to communicate openly with your healthcare providers about all your symptoms and concerns to ensure a comprehensive evaluation. Don't hesitate to seek a second opinion if you feel that your concerns are not being adequately addressed. Your health and well-being are paramount, and finding the right specialist can make a significant difference in your care.
