Suspicious semen

Hello Doctor, this morning after I got up and used the bathroom, while watching TV in the living room, I noticed a patch of liquid on my underwear about the size of a nickel.
I initially thought it was urine, but upon closer inspection, it seemed to be semen.
Later in the evening, while reading in my study, after using the bathroom and returning to read, I felt another patch of liquid on my underwear, about the size of a dime, which also appeared to be semen.
Doctor, if it is semen, should it only occur during nocturnal emissions, or can it happen while awake during the day? Or could it be something else? Thank you.

Dada, 30~39 year old female. Ask Date: 2015/09/05

Dr. Fu Shizhang reply Urology

Nocturnal emissions, also known as wet dreams, primarily occur during sleep but can also happen during the day.
If it is confirmed to be semen, it should be classified as a nocturnal emission, and there is no need for excessive concern.

Experiencing unexplained semen-like discharge can be concerning, and it's important to understand the potential causes and implications. Semen-like discharge can occur for various reasons, and while it may sometimes be normal, it can also indicate underlying health issues that warrant attention.

Firstly, it's essential to differentiate between normal bodily functions and potential medical concerns. In males, semen is typically released during ejaculation, which can occur during sexual activity or nocturnal emissions (commonly referred to as "wet dreams"). However, if you are experiencing this discharge while awake and without any sexual arousal or stimulation, it may not be typical and could indicate a different issue.

One possible explanation for the discharge you are noticing could be pre-ejaculate fluid, which is produced by the Cowper's glands. This fluid can sometimes be released involuntarily, especially if there is heightened arousal or stimulation, even if it is not accompanied by sexual activity. However, if this discharge is occurring frequently and without any sexual context, it may be worth investigating further.

Another potential cause could be an infection or inflammation in the reproductive system. Conditions such as prostatitis (inflammation of the prostate gland) or urethritis (inflammation of the urethra) can lead to unusual discharges. These conditions may also present with other symptoms, such as pain during urination, discomfort in the pelvic area, or changes in urinary habits. If you are experiencing any additional symptoms, it is crucial to seek medical advice.

Sexually transmitted infections (STIs) can also lead to abnormal discharge. Infections such as gonorrhea or chlamydia may present with symptoms that include unusual discharge, burning during urination, or pelvic pain. If there is any possibility of exposure to STIs, it is advisable to get tested.

In some cases, psychological factors such as anxiety or stress can lead to increased awareness of bodily sensations and may contribute to the perception of unusual discharges. However, it is essential to rule out any physical causes before attributing the symptoms to psychological factors.

Given the nature of your symptoms, it is highly recommended that you consult a healthcare professional for a thorough evaluation. A doctor can perform a physical examination, take a detailed medical history, and possibly conduct laboratory tests to determine the cause of the discharge. This may include urine tests, swabs, or blood tests to check for infections or other underlying conditions.

In summary, while occasional semen-like discharge may not be a cause for alarm, persistent or unexplained occurrences should be evaluated by a healthcare provider. Early diagnosis and treatment can help address any potential issues and provide peace of mind. Remember, your health is paramount, and seeking professional advice is always the best course of action when faced with unusual symptoms.
