There is a protrusion on the nipple?


Hello Doctor, I would like to ask about something I noticed today while showering.
I found a painful protrusion on my left nipple, and upon observing it, I see some small white patches.
Compared to other areas of the nipple, this protruding area is more painful.
I had previously noticed a protrusion on the nipple, but it wasn't painful, and its appearance was similar to other areas of the nipple, with only some visual abnormalities.
Since it is just before my menstrual period, I am not sure if this is a sign of breast pathology.
Could you please let me know if this is a precursor to breast disease? Thank you for your help.
