Recurrent glans redness?

Hello, Dr.
Hsu: I previously experienced itching of the penis and sharp pain during erections, so I consulted a doctor who diagnosed me with eczema and prescribed a topical ointment.
However, I don't know why it became much worse; the surface of the glans started to become red, swollen, and even ulcerated.
I then went to another large hospital where they drew blood for tests (one of which came back positive, but the doctor said it likely just indicated a past infection and wasn't a major concern).
They also prescribed me a povidone-iodine ointment, and I felt it was improving, so I switched to using Miconazole or Betamethasone.
However, it would soon relapse, and this cycle has been repeating for nearly a year.
This has really troubled me and made me anxious; I even feel sensitivity and pain in my lower abdomen.
I'm worried that I might have some serious illness.
Please give me some advice, thank you.

A-Zhi: Hello, I am not sure what your exact issue is.
Based on my experience, it may be a localized reaction caused by over-cleaning.
I recommend temporarily discontinuing all medications.
Then, focus on cleaning and caring for the glans.
For cleaning the glans, it is generally advised to use cool clean water only; avoid using hot water, soap, or any cleansing agents such as body wash.
After cleaning, apply petroleum jelly or lotion locally for maintenance.
Try this approach first, and if there are any unclear aspects, feel free to ask again for further discussion and research.
Online responses should only serve as a reference; if necessary, it is still important to seek medical attention at a healthcare facility.

Persistent redness and pain on the glans can be a distressing condition, and it's understandable that you're seeking solutions after experiencing ongoing symptoms for nearly a year. Based on your description, it seems that you have been dealing with a recurrent issue that has not responded well to various treatments, including topical medications like ointments for eczema and antiseptics.

Firstly, it’s important to recognize that the symptoms you are experiencing—itching, pain during erection, redness, and even ulceration—could be indicative of several underlying conditions. While your initial diagnosis was eczema, the persistence and worsening of your symptoms suggest that there may be other factors at play. Conditions such as contact dermatitis, fungal infections (like candidiasis), sexually transmitted infections (STIs), or even inflammatory conditions like balanitis could be contributing to your symptoms.

Given that you have had a positive test result for one of the conditions, it’s crucial to follow up on that with your healthcare provider to ensure that it is not an active infection or a condition that requires specific treatment. The fact that your symptoms improve with certain medications but then recur suggests that there may be an underlying cause that has not been fully addressed.

Here are some steps and considerations that may help you manage your condition:
1. Follow-Up with a Specialist: If you haven't already, consider seeing a urologist or a dermatologist who specializes in skin conditions. They can provide a more thorough examination and may suggest further testing, such as cultures or biopsies, to identify the exact cause of your symptoms.

2. Avoid Irritants: Pay attention to any products you are using in the genital area, including soaps, lotions, and laundry detergents. Sometimes, contact dermatitis can be triggered by irritants or allergens. Switching to hypoallergenic products may help.

3. Maintain Hygiene: Keeping the area clean and dry is essential. After bathing, ensure that the area is thoroughly dried, as moisture can exacerbate fungal infections.

4. Topical Treatments: While you have tried various ointments, it may be beneficial to revisit the use of corticosteroids under the guidance of a healthcare provider, especially if inflammation is significant. Antifungal creams may also be appropriate if a fungal infection is suspected.

5. Consider Systemic Treatments: If topical treatments are ineffective, your doctor may consider systemic medications, especially if an underlying infection or inflammatory condition is diagnosed.

6. Monitor for Other Symptoms: Keep track of any additional symptoms, such as fever, systemic pain, or changes in urination, which could indicate a more serious condition that requires immediate attention.

7. Psychological Support: Chronic pain and discomfort can lead to anxiety and distress. It may be helpful to speak with a mental health professional if you find that your symptoms are affecting your emotional well-being.

8. Lifestyle Modifications: Stress management techniques, such as mindfulness or relaxation exercises, may help alleviate some of the discomfort associated with chronic conditions.

In conclusion, while it can be frustrating to deal with persistent symptoms, a comprehensive approach that includes specialist consultations, careful monitoring, and appropriate treatments can help you find relief. It’s essential to communicate openly with your healthcare providers about your concerns and the impact of these symptoms on your quality of life.
