Shoulder pain?
Hello doctor, I have a back injury.
I went to the hospital today for treatment and received a pain injection.
After the injection, I was asked to wait in the waiting room for five minutes to ensure there were no issues before leaving.
However, on my way home, about 20 to 30 minutes after the injection, I felt that the tightness was becoming more pronounced.
Is this normal?

Dr. Wang Shouji reply Orthopedics
Hello: The feeling of tightness after the injury, along with inflammation and swelling at the site, is likely related to your treatment and not associated with the pain relief injection.
Thank you!

Experiencing shoulder pain or discomfort after receiving pain relief injections can be concerning, and it's important to understand the potential reasons behind this phenomenon. In your case, you mentioned receiving a pain relief injection for a back injury and subsequently feeling an increasing tightness or discomfort in the shoulder area. This situation can be attributed to several factors related to the injection and the underlying condition being treated.
Firstly, it’s essential to recognize that pain relief injections, such as corticosteroids or local anesthetics, are designed to reduce inflammation and alleviate pain. However, the effects of these injections may not be immediate. While some patients experience relief shortly after the injection, others may notice a delay in the onset of pain relief or even an initial increase in discomfort. This can be due to a variety of reasons:
1. Inflammatory Response: After an injection, the body may react with a temporary inflammatory response. This can lead to increased pain or discomfort in the area surrounding the injection site. This phenomenon is sometimes referred to as "rebounding," where patients experience a temporary increase in pain before the medication takes effect.
2. Muscle Tension: The injection itself may cause some muscle tension or irritation in the surrounding tissues. If the injection was administered into or near muscle tissue, it could lead to localized soreness or tightness as the muscles react to the procedure.
3. Underlying Condition: The original condition that prompted the injection may still be contributing to your symptoms. If you have an underlying issue such as a herniated disc, muscle strain, or other musculoskeletal problems, these conditions can continue to cause pain and discomfort even after receiving an injection.
4. Psychological Factors: Pain perception can also be influenced by psychological factors. Anxiety or stress about the injection or the underlying condition can heighten the perception of pain, leading to a feeling of increased discomfort.
Given these factors, it is not uncommon for patients to experience increased tightness or discomfort after receiving a pain relief injection. However, if the pain persists or worsens over the next few days, it is crucial to follow up with your healthcare provider. They can assess your condition, determine if there are any complications, and adjust your treatment plan as necessary.
In the meantime, here are some general recommendations for managing discomfort after an injection:
- Rest: Allow your body to recover by avoiding strenuous activities that may exacerbate the pain.
- Ice Therapy: Applying ice to the affected area can help reduce inflammation and numb the pain.
- Gentle Stretching: Engaging in gentle stretching exercises may help alleviate tightness, but be cautious not to overexert yourself.
- Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may provide additional relief if recommended by your healthcare provider.
In conclusion, while experiencing increased tightness or discomfort after a pain relief injection can be normal, it is essential to monitor your symptoms closely. If the discomfort does not improve or worsens, seeking medical advice is crucial to ensure proper management of your condition. Always communicate openly with your healthcare provider about any concerns you may have regarding your treatment and recovery process.
