There is a protrusion at the site of the fracture sutures?
Hello Doctor, my condition is an ulnar olecranon closed fracture.
It has been two months since my surgery, and the doctor said that the recovery looks good after checking with an X-ray.
However, I have recently noticed that there is a spot at the wound site where the bone feels more prominent, and sometimes when I accidentally bump it against a door, it hurts quite a bit (I still have screws and wires in place).
The doctor mentioned that I could move freely, that the bone healing was okay, and that I should avoid lifting heavy objects.
However, considering that I still cannot fully extend my arm (though I wasn't advised to undergo rehabilitation), along with the recent developments, and since I am studying in another city making it difficult to return for follow-up appointments during the week, I am concerned and want to ask the doctor if my current situation is a normal phenomenon.
Thank you.

Hello: Two months after the ulnar olecranon fracture surgery, you should be able to perform rehabilitation exercises as much as possible.
The prominence at the wound site may be more noticeable after swelling has subsided.
You can visit a hospital in your county to consult with an orthopedic specialist.
Wishing you a speedy recovery.
Best regards, Hong Hsi-Ming.

After an ulnar fracture, particularly one involving the olecranon (the bony prominence of the elbow), it is not uncommon to experience some discomfort or changes in the area surrounding the surgical site. Given that you are two months post-surgery and have noted a protrusion of bone at the site of your fracture, it is important to understand a few key points regarding your recovery and the sensations you are experiencing.
1. Bone Healing and Protrusion: After a fracture, the body goes through a healing process that involves the formation of new bone tissue. This process can sometimes result in areas of bone that feel more prominent or "bumpy" than the surrounding tissue. This is particularly true if there are metal implants, such as screws or plates, involved in your surgery. The presence of these implants can sometimes lead to a sensation of protrusion, especially if the surrounding soft tissue has not fully healed or if there is some swelling.
2. Pain and Sensitivity: The pain you are experiencing when the area is bumped is not unusual. The surgical site can remain sensitive for several months post-operation, particularly if there is still hardware present. The pain can also be exacerbated by activities that involve bending or straightening the elbow, especially if you are still in the process of regaining full range of motion.
3. Range of Motion: You mentioned that you are unable to fully extend your arm. This is a common issue after an olecranon fracture, especially if there was significant damage to the surrounding soft tissues or if the joint was immobilized for an extended period. While your doctor has indicated that you can move freely, it is crucial to engage in rehabilitation exercises to improve your range of motion. If you have not been referred to physical therapy, it may be beneficial to seek out a therapist who can guide you through exercises tailored to your recovery.
4. Follow-Up Care: Since you are studying in a different city and find it challenging to return for follow-up appointments, consider reaching out to your surgeon's office for advice on how to proceed. They may be able to provide guidance over the phone or recommend a local physician who can assess your situation. It is important to have a healthcare professional evaluate the protrusion and any associated pain to rule out complications such as hardware irritation or improper healing.
5. When to Seek Immediate Care: If you experience increased swelling, redness, warmth, or fever, or if the pain becomes severe or changes in nature, it is essential to seek medical attention promptly. These could be signs of infection or other complications that require immediate intervention.
In summary, while some protrusion and discomfort can be normal during the healing process after an olecranon fracture, it is essential to monitor your symptoms closely and engage in rehabilitation to regain full function. If you have concerns about your recovery or the protrusion, do not hesitate to consult with a healthcare professional, either your original surgeon or a local specialist. Your recovery is a priority, and addressing these concerns early can help ensure a better outcome.
