Testicular burning sensation
Hello, Doctor Hsu.
About four days ago, while I was taking a shower, I applied a lot of body wash on my body, particularly on my lower limbs and bladder area, without rinsing the perineum.
I then squatted down to wash my hair and suddenly felt a burning sensation in the lower part of my left scrotum (similar to a solvent burn).
Since I was washing my hair at the time, I thought I could endure it, but afterward, my left testicle and scrotum started to feel hot and painful.
Upon self-examination, there was no swelling, but the left testicle felt slightly harder than the right one.
The burning sensation is still quite pronounced, and the pain radiates to the right testicle, groin, and thigh.
Is this inflammation? I haven't sought medical attention yet; could it possibly be a bacterial infection? (I have had no sexual activity recently, nor have I experienced any cold, fever, or gastrointestinal issues.) Right now, I just feel a constant burning and a dull sensation.
Is medical attention necessary? Could it be varicocele? I'm in a lot of pain.

Dr. Xu Weikai reply Urology
Jie: Hello, it is possible that localized chemical irritation is causing the skin inflammation.
Bacterial infections do not necessarily have to be transmitted through sexual contact.
Bacteria are present everywhere—in the environment, in the air, in water, on hands, and on towels.
After a typical perineal infection, it is recommended to minimize local irritation and apply some topical ointments.
When cleaning, it is not advisable to use soap or body wash; just use cool, clean water.
After cleaning, use lotion or petroleum jelly for local care.
If there are any unclear points, feel free to ask again for further discussion and research.
Online responses should only be used as a reference; if necessary, it is still important to seek medical attention at a healthcare facility.

More Info
Testicular burning sensations can be concerning and may arise from various causes. Based on your description, it seems you experienced a burning sensation in your left scrotum after using a significant amount of body wash without rinsing the area properly. This could potentially lead to irritation or an allergic reaction to the ingredients in the body wash, especially if it contains harsh chemicals or fragrances.
Possible Causes of Testicular Burning
1. Chemical Irritation: As mentioned, the body wash may have caused irritation to the sensitive skin in the genital area. This can lead to a burning sensation, redness, or discomfort. If the product is not rinsed off adequately, it can exacerbate the irritation.
2. Infection: While you mentioned that you have not engaged in sexual activity recently and have no other systemic symptoms like fever or gastrointestinal issues, it's still possible to develop a localized infection. Conditions such as epididymitis (inflammation of the epididymis) or orchitis (inflammation of the testicle) can cause similar symptoms, including burning, pain, and swelling. These infections can be bacterial or viral in nature.
3. Hernia or Testicular Torsion: Although less likely given your description, conditions like an inguinal hernia or testicular torsion can present with pain and discomfort in the testicular area. Testicular torsion is a medical emergency and requires immediate attention.
4. Varicocele: A varicocele is an enlargement of the veins within the scrotum and can cause discomfort or a feeling of heaviness. However, it typically does not present with a burning sensation.
5. Skin Conditions: Conditions such as eczema or fungal infections can also cause burning sensations in the genital area.
Symptoms to Monitor
- Pain: If the pain intensifies or becomes severe, it is crucial to seek medical attention.
- Swelling: While you noted no significant swelling, any changes in size or shape of the testicles should be evaluated.
- Discharge: Any unusual discharge from the penis or changes in urination should be reported to a healthcare provider.
- Fever: The presence of fever may indicate a systemic infection that requires immediate medical intervention.
When to Seek Help
Given the symptoms you are experiencing—persistent burning, discomfort extending to the right testicle, groin, and thigh—it is advisable to seek medical attention. A healthcare provider can perform a physical examination and may order tests such as a urinalysis or ultrasound to determine the underlying cause of your symptoms.
Conclusion
While it is possible that your symptoms are due to irritation from the body wash, the burning sensation and discomfort you describe warrant further evaluation to rule out infections or other conditions. Early diagnosis and treatment can prevent complications and provide relief from your symptoms. Do not hesitate to reach out to a healthcare professional for a thorough assessment.
