Irregular menstruation?

Hello doctor, my last normal menstruation was on June 4th.
The periods in July, August, and September were only light and brownish, not like a regular menstruation.
I have seen a doctor who mentioned it could be ovulation bleeding.
Now, calculating the dates, I should have gotten my period in October, but it is 7 days late.
Excluding the possibility of pregnancy, I would like to know what could be the issue.

Yu, 20~29 year old female. Ask Date: 2015/10/25

Dr. Lü Lizheng reply Obstetrics and Gynecology

The response may indicate irregular menstruation.
There are many causes for irregular periods, and it is advisable to consult a physician for discussion.
Sincerely, Dr.

More Info

Irregular menstruation can be a source of concern for many women, and understanding the potential causes is crucial for addressing any underlying issues. Based on your description, it seems you have experienced a significant change in your menstrual cycle, with lighter and darker-colored bleeding occurring in the months following your last normal period on June 4. The absence of menstruation for seven days past your expected date in October, along with the exclusion of pregnancy, raises several possibilities.

Possible Causes of Irregular Menstruation
1. Hormonal Imbalances: The menstrual cycle is regulated by a complex interplay of hormones, including estrogen and progesterone. Any disruption in the hormonal balance can lead to irregularities. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, or hyperprolactinemia can cause hormonal imbalances that affect menstruation.

2. Ovulation Issues: If ovulation does not occur regularly, it can lead to irregular periods. Anovulation, where the ovaries do not release an egg, can result in missed periods or irregular bleeding. This is often seen in conditions like PCOS.

3. Stress and Lifestyle Factors: Psychological stress, significant weight changes, excessive exercise, or changes in diet can impact your menstrual cycle. Stress can lead to hormonal changes that may delay or alter your period.

4. Age and Menopause: If you are approaching your late 30s or early 40s, perimenopause could be a factor. This transitional phase can cause irregular periods as hormone levels fluctuate.

5. Uterine or Ovarian Conditions: Conditions such as fibroids, polyps, or endometriosis can lead to abnormal bleeding patterns. These conditions can cause changes in the menstrual cycle, including lighter or irregular bleeding.

6. Medications: Certain medications, including hormonal contraceptives, can affect your menstrual cycle. If you have recently started or stopped any medications, this could be a contributing factor.

Concerns and Recommendations
Given your symptoms, it is essential to monitor your menstrual cycle closely. If you continue to experience irregularities, it would be advisable to consult with a healthcare provider for further evaluation. They may recommend:
- Blood Tests: To check hormone levels, thyroid function, and other potential underlying issues.

- Ultrasound: To examine the ovaries and uterus for any abnormalities such as cysts, fibroids, or other conditions.

- Lifestyle Assessment: Discussing your stress levels, exercise habits, and dietary changes can provide insight into potential lifestyle factors affecting your cycle.

Conclusion
Irregular menstruation can stem from various causes, and while some may be benign, others may require medical attention. Since you have already ruled out pregnancy, it is essential to keep track of your symptoms and consult with a healthcare provider if irregularities persist. They can provide a thorough evaluation and guide you toward appropriate management based on your specific situation. Remember, your menstrual health is an important aspect of your overall well-being, and seeking help is a proactive step toward maintaining it.
