Secretions issues?

Hello, doctor: I have a yellow-green discharge that is thick like mucus and has an unpleasant odor.
Sometimes it causes itching.
I would like to inquire about the possible causes and how to treat it.
PS.
I have no sexual experience.
Is a pelvic exam necessary?

Niming, 20~29 year old female. Ask Date: 2015/11/07

Dr. Yang Wangjun reply Obstetrics and Gynecology

Hello: General mucous yellow-green discharge is characteristic of cervicitis.
80% of cases are caused by Chlamydia infection.
In cases of abstinence, a pelvic exam is usually not performed, but a small cotton swab can be inserted into the vagina or a sample can be taken from the vaginal opening for bacterial culture and microscopic examination of the discharge.
Treatment is primarily with tetracycline.

More Info

Abnormal vaginal discharge can be a common concern for many women, and understanding its causes and potential treatments is essential for maintaining reproductive health. Based on your description of yellow-green, thick, and sticky discharge with an unpleasant odor, along with occasional itching, there are several potential causes to consider.

Possible Causes of Abnormal Vaginal Discharge
1. Bacterial Vaginosis (BV): This is one of the most common causes of abnormal vaginal discharge. It occurs when there is an imbalance in the normal bacteria found in the vagina. Symptoms often include a thin, grayish-white discharge with a fishy odor, but some women may experience yellow-green discharge.

2. Yeast Infection: While yeast infections typically cause a thick, white, cottage cheese-like discharge, they can sometimes present with yellowish discharge. Itching and irritation are common symptoms, and the discharge may have a yeasty smell.

3. Sexually Transmitted Infections (STIs): Certain STIs, such as trichomoniasis, can cause yellow-green discharge along with itching and irritation. This type of infection is often accompanied by a foul odor.

4. Cervicitis: Inflammation of the cervix can lead to abnormal discharge, which may be yellow or green and could be associated with pain or discomfort.

5. Hormonal Changes: Fluctuations in hormones, such as those occurring during the menstrual cycle, can also affect the nature of vaginal discharge.

Treatment Options
1. Medical Evaluation: It is crucial to consult a healthcare provider for an accurate diagnosis. A pelvic examination may be necessary to assess the discharge and check for any signs of infection or other abnormalities. A Pap smear or cultures may also be performed to identify the specific cause.

2. Antibiotics: If bacterial vaginosis or an STI is diagnosed, antibiotics will likely be prescribed. It is essential to complete the full course of medication as directed.

3. Antifungal Treatments: If a yeast infection is confirmed, antifungal medications, either topical or oral, will be recommended.

4. Maintaining Hygiene: Keeping the genital area clean and dry can help prevent infections. Avoiding irritants such as scented soaps, douches, and feminine hygiene sprays is also advisable.

5. Probiotics: Some studies suggest that probiotics may help restore the natural balance of bacteria in the vagina, potentially preventing future infections.

Do You Need an Internal Examination?
Given that you have not engaged in sexual intercourse, it is understandable to be hesitant about an internal examination. However, an internal examination can provide valuable information about your reproductive health and help diagnose the cause of your symptoms. It is essential to communicate your concerns with your healthcare provider, who can explain the necessity and benefits of the examination.

Conclusion
In summary, abnormal vaginal discharge can be indicative of various conditions, ranging from infections to hormonal changes. It is essential to seek medical advice for proper diagnosis and treatment. Early intervention can prevent complications and help maintain your reproductive health. Remember, open communication with your healthcare provider is key to addressing your concerns and ensuring you receive the appropriate care.
