Post-Surgery Pain and Care for Anal Fistula Treatment - Surgery

Anal fistula

I had a fistula and partial internal and external hemorrhoid surgery on November 11, and it has been a week since the procedure.
After the surgery, I don't experience much pain when having a bowel movement; I only feel swelling and numbness around the anal area.
By the fifth day post-surgery, the numbness has mostly subsided, and I started to feel some pain at the incision site.
The doctor mentioned that this is normal as the anesthesia wears off, but why does it take so many days for the anesthesia to completely wear off? Is this a normal phenomenon? I noticed that most people seem to experience decreasing pain over time.
Why did I not feel much pain for several days after the surgery, only to start feeling pain on the fifth day? If I need to use the restroom while out and cannot clean the wound properly, can I use commercially available feminine hygiene spray? It also has antibacterial properties.

Anesthesia certainly cannot last for five days without wearing off; any abnormal sensations are likely due to damage to some of the superficial nerves during the surgery, especially in the case of fistula surgery, where such phenomena are more common.
However, this is an unavoidable situation.
Postoperative pain for one to two weeks is normal.
It is better to clean the wound with plain water, as most topical antiseptics can be quite irritating and may adversely affect the wound.

After undergoing surgery for an anal fistula and hemorrhoids, it is common to experience a range of sensations and discomfort as part of the healing process. Your experience of feeling numbness and swelling around the anal area, followed by pain after the fifth day, can be attributed to several factors related to the surgical procedure and the body's natural healing response.

Firstly, the numbness you experienced immediately after surgery is likely due to the local anesthetic used during the procedure. Local anesthetics can take varying amounts of time to wear off, depending on the type used and the individual’s metabolism. It is not unusual for some patients to feel lingering numbness for several days post-surgery. The fact that you felt the numbness diminish by the fifth day indicates that the anesthetic effects were wearing off, which is a normal part of recovery.

As for the onset of pain after the numbness subsided, this is also a typical experience. Post-operative pain can manifest as the anesthetic wears off, and the body begins to react to the surgical trauma. The pain you are feeling is likely a combination of surgical site inflammation, muscle tension, and the healing process. It is important to note that pain levels can fluctuate during recovery, and it is not uncommon for patients to experience discomfort that peaks a few days after surgery before gradually improving.

Regarding your concern about using a commercial feminine wash for cleaning the surgical area, it is generally advisable to follow your surgeon's post-operative care instructions. While some feminine washes may have antibacterial properties, they can also contain fragrances or other ingredients that might irritate the sensitive area around the anus. Instead, it is often recommended to use mild soap and water for cleaning, or to follow any specific cleansing instructions provided by your healthcare provider. If you find yourself in a situation where you cannot clean the area properly, consider using a gentle saline solution or wipes specifically designed for sensitive areas.

In terms of managing post-surgical pain, here are some strategies that may help:
1. Pain Management: Continue to take any prescribed pain medications as directed. Over-the-counter pain relievers like acetaminophen or ibuprofen can also be effective, but consult your doctor before combining medications.

2. Warm Baths: Soaking in a warm bath can help soothe the area, reduce swelling, and alleviate pain. This practice, often referred to as a sitz bath, can be particularly beneficial after bowel movements.

3. Dietary Adjustments: To minimize straining during bowel movements, maintain a high-fiber diet and stay well-hydrated. This can help prevent constipation, which can exacerbate pain and discomfort.

4. Gentle Activity: While rest is important, gentle movement can promote circulation and healing. Avoid heavy lifting or strenuous activities until cleared by your doctor.

5. Follow-Up Care: Keep all follow-up appointments with your surgeon to monitor your recovery. If your pain worsens or if you notice any signs of infection (such as increased redness, swelling, or discharge), contact your healthcare provider immediately.

In summary, the sensations you are experiencing post-surgery are common and can be attributed to the healing process. It is crucial to adhere to your doctor's post-operative care instructions and to communicate any concerns you may have during your recovery. If you have further questions or if your symptoms persist, do not hesitate to reach out to your healthcare provider for personalized advice and support.
