Post-Surgery Concerns: Managing Pain and Discomfort After Fistula Repair - Surgery

It has been almost six months since the fistula surgery?

Hello, Dr.
Ke.
I would like to ask you about my situation.
Last year, I underwent two fistula surgeries.
The first one was in July, and after continuous follow-ups, my attending physician informed me that there was still one area that had not been adequately treated.
Consequently, another surgery was scheduled for November.
Both surgical wounds were quite large and deep.
After the November surgery, the wounds gradually healed normally.
However, I have recently been experiencing significant discomfort and pain in my anal area after bowel movements.
I returned to the outpatient clinic last week, and the doctor mentioned that it might be due to the anal wound not healing completely, which could be causing the pain.
The doctor also stated that during the internal examination, there were no signs of recurrence.
However, a few days later, the pain intensified, and I noticed an increase in discharge, which was not as pronounced during the doctor's examination.
Sometimes, there is even a bit of blood in the discharge.
The doctor prescribed antibiotics for me, and I would like to ask if there is a possibility of recurrence in my case.

Dr. Ke Fangxu reply Surgery

Whether it is a recurrence can only be confirmed through a personal examination by a physician.
If there is an increase in pain or any bleeding, it is important to seek medical evaluation and treatment.

After undergoing two surgeries for fistula repair, it is understandable to be concerned about ongoing pain and discomfort, especially after experiencing significant surgical wounds. The symptoms you describe, including pain during bowel movements, increased discharge, and occasional blood, can be distressing and may indicate several underlying issues.

Firstly, it is essential to recognize that pain after surgery, particularly in the anal region, can be quite common. The area is sensitive, and healing can take time, especially after extensive procedures like fistula repairs. The discomfort you are experiencing may be due to several factors:
1. Incomplete Healing: As your doctor mentioned, if the internal wound has not fully healed, it can lead to pain, especially during bowel movements when the area is under strain. The healing process can vary significantly from person to person, and factors such as infection, inflammation, or even the type of tissue involved can influence recovery time.

2. Scar Tissue Formation: After surgery, scar tissue can form, which may lead to tightness or discomfort in the area. This can sometimes cause pain during bowel movements or when sitting for extended periods.

3. Infection: The presence of increased discharge and blood can be signs of infection. If the surgical site becomes infected, it can lead to increased pain and discomfort. The antibiotics prescribed by your doctor are a standard treatment for this, but if symptoms persist or worsen, further evaluation may be necessary.

4. Fistula Recurrence: While your doctor did not observe any signs of recurrence during the examination, it is still a possibility that should be monitored. Symptoms such as increased pain, discharge, or blood can sometimes indicate that the fistula has returned or that there is another issue at play.

To manage your pain and discomfort effectively, consider the following recommendations:
- Follow-Up Appointments: Continue to attend follow-up appointments with your healthcare provider. If your symptoms worsen or new symptoms develop, inform your doctor immediately. They may recommend imaging studies or further examinations to assess the healing process.

- Pain Management: Discuss pain management options with your doctor. Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help alleviate discomfort. However, always consult your physician before starting any new medication.

- Dietary Adjustments: To ease bowel movements and reduce strain on the surgical site, consider a diet high in fiber. This can help prevent constipation, which can exacerbate pain during bowel movements. Staying hydrated is also crucial.

- Sitz Baths: Taking warm sitz baths can provide relief from pain and discomfort in the anal area. This practice can help soothe the area and promote healing.

- Avoid Straining: During bowel movements, try to avoid straining, as this can put additional pressure on the surgical site. If you experience constipation, consider using stool softeners or fiber supplements as recommended by your doctor.

- Monitor Symptoms: Keep a record of your symptoms, including pain levels, discharge, and any changes you notice. This information can be valuable for your healthcare provider in assessing your condition.

In conclusion, while it is common to experience discomfort after fistula repair surgeries, persistent or worsening symptoms should be addressed promptly. Regular communication with your healthcare provider is essential to ensure proper healing and to rule out any complications. If you have concerns about the possibility of recurrence or if your symptoms do not improve with conservative management, further evaluation may be warranted. Your health and comfort are paramount, and seeking timely medical advice is crucial in navigating your recovery journey.
