Abnormal secretion?
I have been experiencing an increased amount of discharge, which has recently turned yellow with a slight greenish tint.
It has a fishy odor and is thick in consistency.
When I use the restroom, it feels a bit gritty or foamy, and occasionally there seems to be some bleeding mixed with dark red, especially after sexual intercourse.
There is no itching or pain, but I am currently abroad and it's inconvenient for me to seek medical attention.
Could this be inflammation or an infection? What should I do? Thank you very much.
Lin, 20~29 year old female. Ask Date: 2015/11/19
Dr. Lü Lizheng reply Obstetrics and Gynecology
The response indicates that it is likely inflammation.
It is recommended to seek medical treatment.
Sincerely, Dr.

Understanding abnormal vaginal discharge is crucial for identifying potential health issues, especially when accompanied by other symptoms. Based on your description, it seems that you are experiencing an increase in vaginal discharge that is yellowish with a slight green tint, has a fishy odor, and is thick in consistency. Additionally, you mentioned occasional dark red spotting, particularly after sexual intercourse, but without accompanying pain or itching.
There are several potential causes for these symptoms, and they can range from benign to more serious conditions. Here are some possibilities:
1. Bacterial Vaginosis (BV): This is a common condition caused by an imbalance of bacteria in the vagina. It often results in a thin, grayish-white discharge with a fishy odor. While you described your discharge as yellowish, the presence of a strong odor suggests that BV could be a possibility. BV is not typically associated with pain or itching.
2. Yeast Infection: Although yeast infections usually cause itching and irritation, they can sometimes present with a thick, clumpy discharge. However, the odor and color you described do not align perfectly with a typical yeast infection.
3. Sexually Transmitted Infections (STIs): Certain STIs, such as trichomoniasis or chlamydia, can cause abnormal discharge. Trichomoniasis, for example, can lead to a frothy, yellow-green discharge with a foul smell. While you mentioned that you have not experienced pain or itching, it is still essential to consider STIs, especially if there has been any change in sexual partners.
4. Cervical or Uterine Issues: Conditions such as cervicitis (inflammation of the cervix) or polyps can lead to abnormal discharge and spotting. These conditions may not always present with pain but can cause changes in discharge.
Given that you are currently abroad and may not have immediate access to a healthcare provider, here are some steps you can take:
- Maintain Hygiene: Keep the genital area clean and dry. Avoid using scented soaps or feminine hygiene products that can irritate the area.
- Avoid Irritants: Stay away from douches, scented pads, and tampons, as these can disrupt the natural balance of bacteria in the vagina.
- Monitor Symptoms: Keep track of any changes in your discharge, including color, consistency, and odor. Note any new symptoms that may arise, such as itching, burning, or increased spotting.
- Consider Over-the-Counter Treatments: If you suspect a yeast infection, over-the-counter antifungal treatments may provide relief. However, be cautious with self-diagnosis and treatment.
- Seek Medical Attention: As soon as you are able, seek medical attention. A healthcare provider can perform a pelvic examination and tests to determine the exact cause of your symptoms. They may take a sample of the discharge for laboratory analysis, which can help identify infections or other issues.
In conclusion, while your symptoms may suggest an infection or inflammation, it is essential to have a proper diagnosis to receive appropriate treatment. Abnormal vaginal discharge can be a sign of various conditions, and understanding the underlying cause is key to effective management. If you experience any worsening symptoms, such as severe pain, fever, or heavy bleeding, seek medical attention immediately.
