Penile issues
Hello, doctor.
Previously, I had small red spots on the glans and foreskin.
After visiting a large hospital, it was diagnosed as "eczema," and I was prescribed a topical ointment (Dexamethasone) for treatment.
However, a few days later, my penis started to feel a stinging pain, and the foreskin remains red, swollen, and itchy (despite continuous application of the ointment).
It is still red, swollen, and uncomfortable, which is causing me a lot of concern! Thank you for your help!

Dr. Li Xinyi reply Urology
If there is an issue with medication, stop the medication immediately and return to the original physician for evaluation.
If a medication allergy is diagnosed, both the patient and the healthcare facility should document it.

Hello,
It sounds like you're experiencing a challenging situation with your penile health, specifically with symptoms that have been diagnosed as eczema. Eczema, or dermatitis, can manifest in various forms, including allergic reactions, irritant contact dermatitis, or atopic dermatitis, and it can certainly affect sensitive areas such as the genital region.
From your description, it seems that despite using the prescribed topical medication (Dexamethasone, commonly known as "Dun An Fu"), you are still experiencing persistent symptoms, including redness, swelling, itching, and now pain. This can be concerning, and it's understandable that you feel worried.
First, it's important to recognize that while topical corticosteroids like Dexamethasone can be effective for treating eczema, they may not work for everyone, and sometimes they can cause side effects, especially if used for an extended period. Prolonged use can lead to skin thinning, which might exacerbate irritation and discomfort. Additionally, if the underlying cause of the eczema is not addressed, symptoms may persist or worsen.
Here are some considerations and recommendations based on your situation:
1. Follow-Up Consultation: If you haven't already, I strongly recommend scheduling a follow-up appointment with your healthcare provider or a dermatologist. They can reassess your condition and determine if the diagnosis of eczema is accurate or if there might be another underlying issue, such as a fungal infection, bacterial infection, or another dermatological condition.
2. Alternative Treatments: If eczema is confirmed, your doctor may suggest alternative treatments. These could include:
- Topical Calcineurin Inhibitors: Medications like tacrolimus or pimecrolimus can be effective for sensitive areas and may have fewer side effects than corticosteroids.
- Antihistamines: These can help alleviate itching and discomfort.
- Moisturizers: Regularly applying a fragrance-free moisturizer can help maintain skin hydration and barrier function, which is crucial in managing eczema.
3. Avoid Irritants: Identify and avoid potential irritants that could be exacerbating your symptoms. This includes harsh soaps, scented products, and tight-fitting clothing. Opt for loose-fitting, breathable underwear made from natural fibers.
4. Hygiene Practices: Maintain good hygiene, but avoid over-washing the area, as this can strip natural oils and worsen dryness. Use mild, unscented soap and ensure the area is gently dried after washing.
5. Monitor Symptoms: Keep a diary of your symptoms, noting any changes, triggers, or patterns. This information can be valuable for your healthcare provider in determining the best course of action.
6. Consider Psychological Impact: Persistent skin conditions can take a toll on mental health. If you're feeling anxious or stressed about your symptoms, consider discussing this with a mental health professional. They can provide support and coping strategies.
7. Seek Specialist Care: If your symptoms do not improve with the above measures, consider seeking care from a urologist or a dermatologist specializing in genital skin conditions. They may offer more targeted treatments or diagnostic tests to better understand your condition.
In summary, while eczema can be a frustrating condition, there are various treatment options available. It's crucial to work closely with your healthcare provider to find the most effective approach for your specific situation. Don't hesitate to reach out for further assistance or clarification on your treatment plan. Your health and comfort are paramount, and there are solutions available to help you manage these symptoms effectively.
Take care, and I hope you find relief soon.
