Increased discharge and persistent lower abdominal discomfort?
I have been experiencing dull pain in my lower abdomen for the past two months, with an increase in discharge that has a tofu-like consistency and a fishy odor.
However, there is no itching in the external genital area.
Yesterday, my menstrual period started, and when I used a tampon, it absorbed very little blood; mostly, it absorbed discharge.
There was also a significant amount of discharge attached to the tampon, and the blood appeared brown.
What could be the cause of this? Should I go to a gynecologist now or wait until my period is over? Thank you.

Dr. Lin Wenbin reply Obstetrics and Gynecology
If you have vaginal inflammation, do not use tampons.
Please consult a physician for evaluation and treatment.

More Info
Increased discharge and lower abdominal pain in women can be indicative of several underlying conditions, and it is important to understand the potential causes and when to seek medical attention. Based on your description, you are experiencing a few concerning symptoms: persistent lower abdominal pain, an increase in discharge that is described as having a tofu-like consistency and a fishy odor, and abnormal menstrual bleeding characterized by minimal blood flow and brownish color.
Possible Causes
1. Infection: The combination of increased discharge with a foul odor and abdominal pain raises the possibility of a vaginal or pelvic infection. Conditions such as bacterial vaginosis or a sexually transmitted infection (STI) like chlamydia or gonorrhea can cause these symptoms. Bacterial vaginosis, in particular, is known for causing a fishy odor and changes in discharge consistency.
2. Pelvic Inflammatory Disease (PID): If the infection spreads to the reproductive organs, it can lead to PID, which is characterized by lower abdominal pain, abnormal discharge, and sometimes fever. PID can have serious long-term consequences, including infertility, so it is crucial to address it promptly.
3. Hormonal Imbalances: Conditions such as polycystic ovary syndrome (PCOS) can lead to irregular menstrual cycles, increased discharge, and abdominal discomfort. Hormonal fluctuations can also affect the consistency and amount of menstrual flow.
4. Endometriosis: This condition occurs when tissue similar to the lining of the uterus grows outside of it, causing pain, especially during menstruation, and can lead to abnormal bleeding and discharge.
5. Cervical or Uterine Issues: Conditions such as cervical ectropion or fibroids can also lead to increased discharge and abnormal bleeding patterns.
When to Seek Medical Attention
Given the symptoms you are experiencing, it is advisable to seek medical attention sooner rather than later. While it may be tempting to wait until your menstrual period is over, the presence of abnormal discharge and persistent pain suggests that a medical evaluation is warranted. A gynecologist can perform a thorough examination, which may include:
- Pelvic Exam: To check for signs of infection or other abnormalities.
- Ultrasound: To visualize the reproductive organs and identify any structural issues.
- Laboratory Tests: To check for infections, hormonal levels, or other underlying conditions.
Conclusion
In summary, the symptoms you are experiencing—persistent lower abdominal pain, increased discharge with a fishy odor, and abnormal menstrual bleeding—are concerning and warrant prompt medical evaluation. It is essential to address these symptoms early to prevent potential complications and to receive appropriate treatment. Do not hesitate to reach out to a healthcare provider for a thorough assessment and tailored management plan. Early intervention can make a significant difference in outcomes, especially in cases of infections or other reproductive health issues.
