Experiencing discomfort or itching upon contact with the glans (head of the penis) may indicate an underlying condition?
Due to my phimosis, the glans only gradually became exposed during an erection around the age of 21.
However, the area around the glans is slightly red but not swollen, and there are some small wrinkles.
The glans feels uncomfortable when it comes into contact with the fabric of my underwear or when water hits it while showering (it feels itchy but not painful).
Will this condition improve?

More Info
Discomfort and itching around the glans can be a common issue, especially in individuals with conditions such as phimosis (a condition where the foreskin cannot be easily retracted over the glans). Your description suggests that you may be experiencing irritation due to the friction caused by clothing or water exposure, which can lead to redness and discomfort.
Possible Causes
1. Phimosis: As you mentioned, having a long foreskin can lead to difficulty in exposing the glans. This can cause irritation and discomfort, especially when the glans is exposed to friction from clothing or during bathing.
2. Irritation or Allergic Reaction: The materials of your underwear or soaps used during bathing may cause irritation or an allergic reaction, leading to redness and itching.
3. Infections: Conditions such as candidiasis (yeast infection) or bacterial infections can also cause discomfort and itching. These infections can occur more frequently in individuals with phimosis due to moisture retention under the foreskin.
4. Dermatitis: Contact dermatitis from soaps, detergents, or other irritants can lead to inflammation and discomfort around the glans.
5. Poor Hygiene: Inadequate cleaning of the area can lead to the buildup of smegma (a combination of dead skin cells, oils, and moisture), which can cause irritation.
Solutions
1. Improved Hygiene: Ensure that you are cleaning the area thoroughly but gently. If you can retract the foreskin, clean underneath it with mild soap and water to prevent the buildup of smegma.
2. Moisturizing: Applying a gentle, fragrance-free moisturizer can help soothe the skin and reduce irritation. Avoid using harsh soaps or products with fragrances that may exacerbate the issue.
3. Avoiding Irritants: Consider switching to breathable cotton underwear and avoid tight-fitting clothing that may cause friction. Also, be cautious with soaps and detergents that may irritate the skin.
4. Topical Treatments: Over-the-counter hydrocortisone cream may help reduce inflammation and itching. However, it’s best to consult a healthcare provider before using any topical treatments.
5. Consult a Healthcare Provider: If the symptoms persist or worsen, it is crucial to consult a healthcare provider. They can assess whether there is an underlying infection or other condition that needs treatment. In some cases, a referral to a urologist may be necessary, especially if phimosis is causing significant discomfort.
6. Consider Circumcision: If phimosis is the underlying cause of your discomfort and it is severe, circumcision may be recommended. This surgical procedure removes the foreskin, which can alleviate the symptoms associated with phimosis and improve hygiene.
Conclusion
While your symptoms may improve with proper hygiene and care, it is essential to monitor the situation closely. If you notice any changes, such as increased redness, swelling, or discharge, seek medical attention promptly. Remember, it is always better to err on the side of caution when it comes to health concerns, especially in sensitive areas.
