Prepuce issues (inquiries regarding the foreskin)
Hello, doctor.
I am unable to retract my foreskin back to the coronal sulcus during an erection without manual assistance.
Is this a concern? Additionally, when I do retract it to the coronal sulcus, it causes some pain.
Could this be related to the frenulum? I also notice that the glans appears to sag slightly when I pull it back.
BGT, 20~29 year old female. Ask Date: 2015/12/15
Dr. Xu Weikai reply Urology
BGT: Hello, as long as there is no phimosis, the foreskin can be easily retracted, and generally, surgery is not required.
Unless you are very concerned about the length of the foreskin, or if the foreskin causes pain during an erection.
If there are any unclear points, feel free to ask again, and we can discuss and research further.

When it comes to concerns about foreskin issues, particularly regarding retraction and pain during an erection, it's essential to understand the anatomy and potential conditions that could be affecting you. Based on your description, it seems you are experiencing difficulty with the natural retraction of the foreskin to the coronal sulcus (the groove at the base of the glans) during an erection, and you also mention pain when attempting to retract it manually.
Firstly, it is important to note that the foreskin is a sensitive part of the male anatomy, and its ability to retract can vary significantly among individuals. In some cases, the foreskin may not retract fully due to a condition known as phimosis, where the foreskin is too tight to be pulled back over the glans. This can lead to discomfort, especially during erections, as the increased size of the glans can cause the foreskin to stretch uncomfortably.
The pain you experience when retracting the foreskin could be attributed to several factors:
1. Phimosis: As mentioned, if the foreskin is too tight, it may cause pain when you attempt to retract it. This condition can be congenital or develop over time due to scarring or inflammation.
2. Frenulum Issues: The frenulum is the band of tissue on the underside of the penis that connects the foreskin to the glans. If this tissue is too short or tight, it can cause pain during retraction and may lead to a condition known as frenulum breve. This could explain the discomfort you feel when trying to pull back the foreskin.
3. Infection or Inflammation: Conditions such as balanitis (inflammation of the glans) or posthitis (inflammation of the foreskin) can also cause pain and discomfort. These conditions may arise from poor hygiene, infections, or irritants.
4. Sensitivity of the Glans: If the glans has not been exposed regularly, it may be sensitive to touch, leading to discomfort when the foreskin is retracted.
Given these possibilities, it is crucial to consult a healthcare professional, preferably a urologist, who can perform a physical examination and provide a proper diagnosis. They may recommend treatments ranging from topical steroids to help with phimosis, to surgical options like circumcision if necessary.
In the meantime, here are some general recommendations:
- Gentle Retraction: If you are able to retract the foreskin manually, do so gently and avoid forcing it, as this can lead to further injury or scarring.
- Hygiene: Maintain good hygiene by cleaning the area regularly to prevent infections, which can exacerbate discomfort.
- Moisturizing: Using a water-based lubricant or a gentle moisturizer may help reduce friction and discomfort when retracting the foreskin.
- Avoid Irritants: Be mindful of soaps or products that may irritate the sensitive skin of the glans or foreskin.
In conclusion, while some variation in foreskin retraction is normal, persistent pain and difficulty should not be ignored. Seeking medical advice is the best course of action to ensure that any underlying issues are addressed appropriately.
