Palpitations?
Hello, Doctor.
I have been exercising frequently since elementary school, mainly focusing on cardiovascular endurance activities like jogging and swimming.
However, during middle school, I started to notice that my heart was beating more forcefully and I experienced some chest tightness.
When I asked my father about it, he said it was normal, so I didn't think much of it.
Now that I'm in college, I suddenly feel that my heart is beating even harder, and the chest tightness has increased, making me feel uncomfortable.
I would like to ask if this could be considered palpitations? Should I schedule an appointment, or would it be sufficient to just rest more? Thank you, Doctor.

Dear Patient,
Thank you for reaching out with your concerns regarding your heart palpitations and chest discomfort. It's important to address these symptoms seriously, especially given your history of regular exercise and the changes you've noticed over time.
Palpitations, or the sensation of feeling your heart beating, can be caused by a variety of factors. They can range from benign to more serious underlying conditions. In your case, since you have been experiencing these symptoms since middle school and they have intensified recently, it is essential to consider a few key points.
1. Nature of Symptoms: You mentioned that your heart feels like it is beating harder and that you are experiencing increased chest tightness. Palpitations can feel like your heart is racing, pounding, or fluttering. If these sensations are accompanied by other symptoms such as dizziness, shortness of breath, or fainting, it is crucial to seek medical attention promptly.
2. Potential Causes: There are many potential causes of palpitations, including:
- Physiological Factors: Stress, anxiety, caffeine intake, or dehydration can lead to palpitations. Given that you are a college student, stress from studies or lifestyle changes could be contributing factors.
- Cardiac Issues: Conditions such as arrhythmias, which are irregular heartbeats, can cause palpitations. Other cardiac conditions, such as structural heart problems or ischemic heart disease, may also present with similar symptoms.
- Hormonal Changes: Fluctuations in hormones, particularly in younger adults, can also lead to palpitations.
3. When to Seek Medical Advice: Given that your symptoms have worsened, it would be advisable to consult a healthcare professional. You should consider scheduling an appointment if:
- The palpitations are frequent or persistent.
- You experience any additional symptoms such as chest pain, severe shortness of breath, dizziness, or fainting.
- You have a family history of heart disease or other cardiovascular conditions.
4. Diagnostic Approach: During your visit, your doctor may perform a physical examination and recommend tests such as an electrocardiogram (ECG) to assess your heart's rhythm, or possibly a Holter monitor to track your heart activity over a period of time. Blood tests may also be conducted to check for thyroid function or electrolyte imbalances.
5. Management and Lifestyle Adjustments: While waiting for your appointment, consider monitoring your symptoms. Keeping a diary of when the palpitations occur, their duration, and any associated activities or stressors can provide valuable information to your healthcare provider. Additionally, reducing caffeine intake, managing stress through relaxation techniques, and ensuring adequate hydration may help alleviate some symptoms.
In summary, while occasional palpitations can be normal, the increase in intensity and associated chest discomfort warrants a medical evaluation. It is always better to err on the side of caution when it comes to heart health. Please do not hesitate to seek medical advice to ensure your well-being.
Wishing you good health,
