Regarding vaginal bleeding?

Hello doctor, I have been experiencing vaginal inflammation and I'm not sure if the pain is due to excessive wiping after using the restroom, which may have caused some abrasions.
Last night, I noticed some bleeding, so I went to see a gynecologist the next day.
I was prescribed a three-day course of pills, ointment, and suppositories.
After using the suppository on the second day, I applied the ointment (using a cotton swab for both internal and external application) and noticed that the cotton swab was saturated with blood (which was not the case on the first day).
While applying the ointment, I also felt what seemed like a small abrasion inside (with mild pain).
After the suppository dissolved, I noticed clear bleeding when wiping after using the restroom.
I'm unsure if I'm using the medication incorrectly or if my condition has worsened.
Since today is a holiday, I cannot return for a follow-up appointment, and I haven't found any similar cases online, which makes me very worried.

Possible causes of bleeding include the vaginal surface, cervical opening, and uterine cavity.
If the bleeding originates from the vaginal surface, it may be due to inflammation or abrasions, which typically results in minimal bleeding.
However, it is important to identify the cause of the inflammation and treat the underlying pathogens.
The cervical opening may also bleed due to inflammation and abrasions, usually with minimal bleeding as well.
Again, determining the cause of the inflammation and addressing the pathogens is essential.
If the bleeding is from the uterine cavity, there are several potential causes, including irregular menstruation, inflammation, tumors, or pregnancy, all of which require specific treatment based on the underlying cause.
If the bleeding is minimal, there is no need for immediate concern, and a follow-up appointment can be scheduled for the next day.
However, if there is significant bleeding, it is necessary to seek emergency care at a gynecology department in a major hospital.
Sincerely, Dr.

More Info

Vaginal bleeding can be a concerning symptom for many women, and understanding its causes, symptoms, and treatment options is essential for proper management. In your case, it seems that you are experiencing vaginal bleeding following a diagnosis of vaginitis, which may have been exacerbated by irritation from excessive wiping.

Causes of Vaginal Bleeding
1. Vaginal Infections: Conditions like vaginitis, which can be caused by bacterial infections, yeast infections, or sexually transmitted infections, can lead to inflammation and irritation of the vaginal tissues. This inflammation can cause bleeding, especially if there is any trauma or excessive friction.

2. Cervical Issues: The cervix can also be a source of bleeding. Conditions such as cervicitis (inflammation of the cervix) or cervical polyps can lead to bleeding, particularly after intercourse or during examinations.

3. Hormonal Fluctuations: Hormonal imbalances can lead to irregular bleeding. This is particularly common in women who are approaching menopause or those with conditions like polycystic ovary syndrome (PCOS).

4. Trauma: As you mentioned, excessive wiping or inserting objects can cause micro-tears in the vaginal lining, leading to bleeding. This is often accompanied by pain or discomfort.

5. Other Medical Conditions: Conditions such as fibroids, endometriosis, or even early pregnancy complications can also result in abnormal bleeding.

Symptoms to Monitor
In addition to bleeding, you should be aware of other symptoms that may indicate a more serious condition:
- Severe Pain: If you experience significant pain, it may indicate a more serious issue.

- Foul Odor: A strong, unpleasant odor accompanying the discharge may suggest an infection.

- Fever: A fever may indicate an infection that requires immediate medical attention.

- Heavy Bleeding: If you soak through a pad or tampon in an hour or less, seek emergency care.

Treatment Options
1. Medications: Depending on the cause of your bleeding, your doctor may prescribe antibiotics for infections, hormonal treatments for imbalances, or anti-inflammatory medications for pain management.

2. Home Care: If the bleeding is due to minor trauma, it may resolve on its own. You can manage discomfort with warm baths, avoiding irritants (like scented soaps), and using over-the-counter pain relief if necessary.

3. Follow-Up Care: Since you have already seen a gynecologist, it is important to follow up with them as soon as possible, especially if the bleeding continues or worsens. They may recommend further testing, such as a pelvic exam or ultrasound, to rule out other causes.

4. Avoiding Irritation: Until you can see your doctor, avoid activities that may irritate the vaginal area, such as douching, using tampons, or engaging in sexual intercourse.

Conclusion
While it is understandable to feel anxious about vaginal bleeding, especially when it is accompanied by pain, many causes are treatable. It is crucial to communicate openly with your healthcare provider about your symptoms, including any activities that may have contributed to the bleeding. If you notice any alarming symptoms or if the bleeding persists, do not hesitate to seek medical attention. Your health and peace of mind are paramount.
