Anal itching and persistent difficulty in cleaning after bowel movements?

Hello, doctor.
I had surgery for an anal abscess six months ago.
After recovering from the surgery, I now have a small flesh-colored bump at the anal opening, about the size of a red bean.
What could this be? Additionally, after each bowel movement, I rinse with water or use moist wipes to ensure cleanliness before leaving.
However, about half an hour later, I start to feel itching around the anus.
When I go back to the restroom and wipe with toilet paper, I find that I haven't cleaned thoroughly.
I have normal bowel movements every day, but I experience occasional itching, especially after a bowel movement.
What could be the issue?

Dr. Ke Fangxu reply Surgery

After anal abscess surgery, some individuals may develop an anal fistula.
If this "fleshy lump" occasionally experiences swelling, pain, and discharge, the likelihood of an anal fistula is quite high, and it is advisable to consult a physician.
As for anal itching, it is a common symptom associated with hemorrhoids.
Topical ointments can be applied, which should help alleviate the itching.
The issue of seepage should also improve once the itching is resolved.

Anal itching and post-bowel movement discomfort can be quite distressing, especially for someone who has undergone surgery for an anal abscess. The symptoms you are experiencing, including the presence of a small bump near the anus and persistent itching, can be attributed to several potential causes.

Firstly, the small bump you described, which is about the size of a red bean, could be a number of things. It might be a skin tag, which is a benign growth that can occur after surgery or due to friction in the area. Skin tags are usually harmless but can sometimes become irritated. Another possibility is that it could be a hemorrhoid, which is a swollen vein in the anal area that can cause discomfort, itching, and sometimes bleeding. Given your surgical history, it’s also possible that this bump could be a remnant of the abscess or a related complication, such as a fistula, which is an abnormal connection between the anal canal and the skin.

Regarding the itching you experience after bowel movements, this can be caused by several factors. One common reason is inadequate cleaning after a bowel movement. Even if you feel clean, residual stool can irritate the sensitive skin around the anus, leading to itching. It’s good that you are using water and moist wipes, but you might want to ensure that you are thoroughly cleaning the area without causing irritation. Sometimes, using a gentle, fragrance-free soap can help, but be cautious not to over-clean, as this can also lead to irritation.

Another potential cause of anal itching is moisture. If the area remains damp after cleaning, it can create an environment conducive to irritation or fungal infections. Ensuring that the area is dry after washing can help alleviate this issue. You might consider using a soft towel or allowing the area to air dry before dressing.

Dietary factors can also play a role in anal itching. Certain foods, such as spicy foods, caffeine, and alcohol, can irritate the anal area and lead to discomfort. Keeping a food diary to track any correlations between what you eat and your symptoms might be beneficial.

In some cases, anal itching can be a sign of a skin condition, such as eczema or psoriasis, which can affect the anal area. If you have a history of skin conditions, this might be worth considering.

Given your symptoms and surgical history, it’s essential to consult with a healthcare professional for a thorough evaluation. They may perform a physical examination to determine the nature of the bump and assess for any signs of infection or other complications. Depending on their findings, they may recommend treatments such as topical creams to reduce itching or inflammation, dietary changes, or further diagnostic tests if necessary.

In summary, while the symptoms you are experiencing can be common and often benign, they warrant a professional evaluation, especially considering your surgical history. Maintaining good hygiene, ensuring the area is dry, and being mindful of your diet can help manage your symptoms. However, a healthcare provider will be able to give you the most accurate diagnosis and appropriate treatment options.
