Acne and skin issues?
Hello, doctor.
I have been troubled by my skin issues for a long time.
I often have bumps on my face and limbs that resemble acne, but they are not blackheads or pustules; the surface is very smooth.
When squeezed, they release an oily fluid.
There are also some that become visible only when the skin is stretched, appearing as small dark bumps.
Sometimes, they become raised and when squeezed, they contain many tiny, hard, hair-like substances.
I would like to ask how I can improve this condition.
Is it related to my diet or my cleansing routine? I previously consulted a dermatologist who said to just wait for them to come out and then squeeze them, indicating it was not a serious issue.
I would appreciate your response.
Thank you.

It is likely a cyst; if it is very bothersome, you may consider surgical removal.

More Info
Understanding acne and various skin issues can be quite complex, as they can stem from a multitude of factors including genetics, hormonal changes, diet, and environmental influences. Based on your description, it sounds like you are experiencing a type of acne that may not fit the typical presentation of blackheads or pustules, but rather a form of clogged pores or possibly even a condition known as sebaceous hyperplasia, which can appear as small, smooth bumps on the skin.
Causes of Your Skin Issues
1. Sebaceous Glands: The skin contains sebaceous glands that produce oil (sebum) to keep the skin moisturized. When these glands become overactive, they can lead to clogged pores, resulting in the formation of acne-like bumps. The "oil" you describe when you squeeze these bumps could be the sebum that has accumulated in the clogged pore.
2. Diet: There is ongoing research into the relationship between diet and acne. High glycemic index foods (like white bread and sugary snacks) and dairy products have been associated with increased acne in some individuals. Keeping a food diary to track any correlations between your diet and skin flare-ups might be beneficial.
3. Hormonal Fluctuations: Hormones play a significant role in acne development. Fluctuations, especially during puberty, menstrual cycles, or due to stress, can increase oil production and lead to breakouts.
4. Skin Care Routine: Over-cleansing or using harsh products can strip the skin of its natural oils, prompting the sebaceous glands to produce even more oil, which can exacerbate the problem. Conversely, not cleansing adequately can lead to the buildup of dead skin cells and oil, contributing to clogged pores.
5. Environmental Factors: Humidity, pollution, and exposure to certain chemicals can also affect skin health. If you live in a hot and humid climate, sweat and oil can mix and clog pores, leading to breakouts.
Solutions and Recommendations
1. Gentle Cleansing: Use a mild, non-comedogenic cleanser twice daily to remove excess oil and dirt without irritating your skin. Avoid scrubbing too hard, as this can worsen inflammation and lead to more breakouts.
2. Moisturizing: Even if your skin is oily, it is essential to use a lightweight, oil-free moisturizer to maintain hydration. This can help balance oil production and prevent the skin from becoming overly dry.
3. Topical Treatments: Over-the-counter products containing salicylic acid or benzoyl peroxide can help treat and prevent acne. Salicylic acid helps to exfoliate the skin and keep pores clear, while benzoyl peroxide kills acne-causing bacteria.
4. Avoid Picking: It’s crucial to avoid squeezing or picking at the bumps, as this can lead to scarring and further irritation.
5. Dietary Adjustments: Consider reducing your intake of high-glycemic foods and dairy to see if your skin improves. Incorporating more fruits, vegetables, and whole grains can also promote overall skin health.
6. Consult a Dermatologist: Since you have already seen a dermatologist, it might be worth following up if your condition does not improve. They may recommend prescription treatments such as topical retinoids, which can help unclog pores and reduce the formation of new acne.
7. Patience and Consistency: Skin treatments often take time to show results. It’s important to be patient and consistent with your skincare routine.
In summary, your skin issues could be attributed to a combination of factors including oil production, diet, and skincare habits. By adopting a gentle skincare routine, making dietary adjustments, and possibly seeking further dermatological advice, you can work towards improving your skin condition. Remember, skin health is a journey, and what works for one person may not work for another, so it’s essential to find a routine that suits your individual needs.
