Epididymitis issue, it's been a long time. Update?
Hello, I first developed symptoms in September 2014.
After receiving injections and taking antibiotics, I recently had an ultrasound last month, which indicated that I have epididymitis on the left side.
Despite receiving intravenous treatment and medication, my condition has not improved; the pain is only slightly less than before.
Am I experiencing chronic epididymitis? How should I manage this? Is it treatable? I have undergone all necessary tests, and there were no bacteria found, and my semen analysis was normal.
My blood tests also came back fine.
However, the ultrasound results indicated epididymitis, and I experience pain about five times a day on both sides.
I have been on medication and received injections for over a year now.
A few days ago, I felt relatively fine, but today the pain has returned, albeit mild.
I am unsure what is happening.
I have been on medication to the point where even the doctor advised stopping, and it has been quite difficult.
I am also concerned about whether this will affect my fertility.
Additionally, I have some discomfort on the glans.
What should I do? Doctor, I currently feel a sense of fullness with mild soreness, but it does not hurt when I touch it.

Dr. Xu Weikai reply Urology
Xiao Liang: Hello, if you are still experiencing pain, it may indicate an acute flare-up.
Epididymitis typically follows this course.
It will not completely return to its original size; it is usually larger, about 1 to 2 times the original size.
In terms of diet, it is important to avoid irritating foods: refrain from smoking and alcohol, and avoid scallions, ginger, garlic, chili peppers, black pepper, fried foods, and excessively oily or salty foods.
Additionally, drink plenty of water to maintain a daily urine output of around 1500 cc.
It is also important to maintain a regular schedule and avoid staying up late.
If you have any further questions, feel free to ask, and we can discuss and research together.
Online responses should only be used as a reference; if necessary, it is important to seek medical attention at a healthcare facility.

More Info
Chronic epididymitis is a condition characterized by inflammation of the epididymis, which is a coiled tube located at the back of the testicle that stores and carries sperm. This condition can be quite distressing and is often associated with various symptoms, including pain, swelling, and discomfort in the affected area. Understanding the symptoms and treatment options is crucial for managing this condition effectively.
Symptoms of Chronic Epididymitis
The symptoms of chronic epididymitis can vary from person to person, but common signs include:
1. Persistent Pain: Patients often report a dull, aching pain in the scrotum or the lower abdomen. This pain can fluctuate in intensity and may be exacerbated by physical activity or prolonged sitting.
2. Swelling: The affected epididymis may become swollen, leading to a noticeable enlargement of the scrotum.
3. Discomfort During Sexual Activity: Many individuals experience discomfort or pain during sexual intercourse or ejaculation.
4. Sensitivity: The area may be sensitive to touch, although some patients report that the pain is not severe when palpated.
5. Associated Symptoms: In some cases, there may be accompanying symptoms such as fever, chills, or urinary issues, although these are more common in acute cases.
Causes and Diagnosis
Chronic epididymitis can arise from various causes, including:
- Infections: While acute epididymitis is often caused by bacterial infections (such as sexually transmitted infections), chronic cases may not always have a clear infectious origin. In your case, you mentioned that multiple tests have shown no bacterial presence, which is not uncommon in chronic cases.
- Non-infectious Factors: Other potential causes include trauma, prolonged sitting, or even autoimmune conditions.
Given that your ultrasound has confirmed the presence of epididymitis, and you have undergone various tests with no significant findings, it is likely that you are dealing with chronic epididymitis. This condition can sometimes be challenging to treat, especially when the underlying cause is not clear.
Treatment Options
1. Medications: Since you have already been on antibiotics and pain medications, it may be worth discussing with your healthcare provider the possibility of switching to a different class of medications. Non-steroidal anti-inflammatory drugs (NSAIDs) can help manage pain and inflammation.
2. Physical Therapy: Some patients find relief through pelvic floor physical therapy, which can help alleviate discomfort associated with chronic pelvic pain syndromes.
3. Surgery: In rare cases, if conservative treatments fail and the pain is debilitating, surgical options may be considered. This could involve procedures to remove the affected epididymis or other interventions.
4. Lifestyle Modifications: Avoiding activities that exacerbate symptoms, such as heavy lifting or prolonged sitting, may help. Wearing supportive underwear can also provide comfort.
5. Follow-up Care: Regular follow-up with your healthcare provider is essential to monitor your condition and adjust treatment as necessary. Given your history of fluctuating symptoms, it is important to keep an open line of communication with your doctor.
Impact on Fertility
Chronic epididymitis can potentially affect fertility, particularly if there is significant damage to the epididymis or if sperm transport is compromised. However, many men with chronic epididymitis maintain normal fertility. It is advisable to discuss your concerns about fertility with your healthcare provider, who may recommend a semen analysis to assess sperm health.
Conclusion
Chronic epididymitis can be a frustrating condition, particularly when symptoms persist despite treatment. It is essential to work closely with your healthcare provider to explore all available treatment options and to monitor your symptoms closely. While the condition can be challenging, many patients find relief through a combination of medical management and lifestyle adjustments. If you have ongoing concerns or if your symptoms worsen, do not hesitate to seek further evaluation or a second opinion.
