Traditional mole excision surgery and circumferential excision surgery?

Hello Dr.
Yu,
Next week I will undergo a mole excision surgery at National Taiwan University Hospital in Yunlin, but I still feel anxious.
I am unsure whether to choose traditional surgery or PPH (Procedure for Prolapse and Hemorrhoids) surgery.
As a working professional, I hope to minimize pain and recover quickly so I can return to work.
The doctors at NTU suggested using PPH surgery, but I am concerned that the recurrence and complications associated with PPH surgery are higher than those of traditional surgery.
On the other hand, I worry that I may not recover quickly enough from traditional surgery.

Could you help me analyze which surgical option would be more appropriate? Also, is it necessary to have family or friends present at the hospital during the surgery? If I do not have anyone there, will the hospital still proceed with the surgery?

Dr. Yu Mingchang reply Surgery

Hello: Traditional methods are sufficient; one person can be in the hospital.

When it comes to choosing between traditional surgery and PPH (Procedure for Prolapse and Hemorrhoids) surgery for skin lesions, there are several factors to consider, especially regarding your concerns about pain, recovery time, and the potential for complications.

Understanding the Surgical Options:
1. Traditional Surgery: This method typically involves excising the lesion with a scalpel. It is a well-established technique with a long history of use. The advantages of traditional surgery include:
- Lower Recurrence Rates: Traditional excision generally has lower rates of recurrence for skin lesions compared to some newer methods.

- Clear Margins: Surgeons can ensure that the lesion is completely removed, which is crucial for preventing recurrence.

- Familiarity: Most surgeons are highly experienced with this technique, which can lead to more predictable outcomes.

However, traditional surgery can come with:
- Longer Recovery Time: The recovery period may be longer, and you might experience more postoperative pain and discomfort.

- Larger Scars: Depending on the size and location of the lesion, the incision may be larger, leading to more noticeable scarring.

2. PPH Surgery: This technique is less invasive and is often associated with quicker recovery times. The benefits include:
- Minimally Invasive: PPH is designed to reduce tissue trauma, which can lead to less postoperative pain and a faster return to normal activities.

- Smaller Incisions: The incisions made during PPH are generally smaller, which may result in less scarring.

However, there are some drawbacks:
- Higher Recurrence Rates: Some studies suggest that PPH may have higher rates of recurrence for certain types of lesions.

- Potential Complications: As with any surgical procedure, there are risks involved, and PPH may have specific complications that are less common with traditional surgery.

Making the Decision:
Given your concerns about returning to work quickly, PPH may seem appealing due to its minimally invasive nature and shorter recovery time. However, if the lesion is one that has a higher risk of recurrence with PPH, you might want to weigh that against the potential for a longer recovery with traditional surgery.

It is essential to have a thorough discussion with your surgeon about the specifics of your case, including the type of lesion, its location, and your overall health. They can provide insights based on their experience and the latest research, helping you make an informed decision.

Regarding Support During Surgery:
As for having a friend or family member present during the surgery, it is generally not a requirement for the procedure itself. Most hospitals will proceed with the surgery even if you do not have someone accompanying you. However, having someone there can provide emotional support and assistance during your recovery, especially if you experience any discomfort or need help getting home afterward. It’s advisable to check with the hospital regarding their policies on this matter.

In conclusion, the choice between traditional and PPH surgery should be made based on a careful consideration of the benefits and risks associated with each method, as well as your personal circumstances and preferences. Always consult with your healthcare provider to ensure that you are making the best choice for your health and lifestyle.
