Lochia after abortion

Hello Dr.
Tian: I was 8 weeks pregnant on January 2nd, but the embryo had no heartbeat, and I underwent a dilation and curettage (D&C) procedure.
Today is the sixth day post-surgery.
Over the past two days, I've noticed a decrease in bleeding, but the blood is dark brown in color (both on the sanitary pad and when wiping).
Additionally, I occasionally experience abdominal pain.
Is the color of the blood and the abdominal pain normal? Should I schedule a follow-up appointment with my original physician, or is it sufficient to wait until the lochia clears up? Thank you for your response.

JJ, 30~39 year old female. Ask Date: 2016/01/08

Dr. Tian Yiwen reply Obstetrics and Gynecology

If the amount of bleeding decreases and the color becomes darker, resembling the end of a menstrual period, it is likely not a significant issue.
However, if abdominal pain intensifies, it is advisable to consult your physician for a follow-up appointment.

More Info

Post-abortion bleeding, often referred to as lochia, is a common experience following a miscarriage or abortion. It is essential to understand the normal progression of bleeding and the associated symptoms to determine whether further medical evaluation is necessary.
In your case, you mentioned that it has been six days since your abortion at eight weeks of pregnancy, and you are observing a decrease in blood volume with a change in color to a dark brown or "black sugar" hue. This color change is typically indicative of older blood, which is common as the body begins to clear out the uterine lining and any remaining tissue. Initially, post-abortion bleeding may present as bright red, similar to a menstrual period, but as the days progress, it can transition to darker shades as the bleeding slows down and the body continues its healing process.

Regarding the abdominal pain you are experiencing, it is not uncommon to have some level of discomfort following an abortion. This pain can be attributed to the uterus contracting as it returns to its pre-pregnancy size and expels any remaining tissue. However, if the pain is severe, persistent, or worsening, it could indicate complications such as retained products of conception, infection, or other issues that may require medical attention.

Here are some key points to consider regarding your situation:
1. Color of Bleeding: The transition from bright red to dark brown or black is generally a normal part of the healing process. However, if you notice any bright red bleeding returning after it has turned dark, or if the bleeding becomes excessively heavy (soaking through a pad in an hour), you should seek medical attention.

2. Abdominal Pain: Mild cramping or discomfort can be expected, but if you experience sharp, severe pain, or if the pain is accompanied by fever, chills, or foul-smelling discharge, these could be signs of an infection or other complications. In such cases, it is advisable to consult your healthcare provider.

3. Follow-Up Care: It is essential to have a follow-up appointment with your healthcare provider after an abortion to ensure that your body is healing correctly. If you have concerns about your symptoms or if they do not improve, it is prudent to reach out to your doctor for further evaluation.

4. Monitoring Symptoms: Keep track of your bleeding patterns and any changes in pain. If your symptoms worsen or if you develop new symptoms, do not hesitate to seek medical care.
In summary, while some bleeding and abdominal discomfort are normal after an abortion, it is crucial to monitor your symptoms closely. If you have any doubts or if your condition changes, contacting your healthcare provider for advice and possible follow-up is the best course of action. Your health and well-being are paramount, and ensuring that you receive appropriate care is essential for your recovery.
