Red spots on the glans?

Last Sunday, I was not very careful and had some fun with my girlfriend in a public restroom.
Since I didn't have a condom, I ended up ejaculating inside her.
After that, on Tuesday, I noticed some red spots on my glans, and by Wednesday, it felt a bit itchy.
I'm not sure if I might have contracted something because during a check-up in middle school, the doctor mentioned that I have a long foreskin.
I wonder if there's any connection.

Dr. Li Yilun reply Urology

You are very young, and it is not appropriate to engage in sexual activity at an early age.
If your partner is under 16 years old, there may be legal issues involved.
It is important to understand that a simple description cannot determine your condition.
Therefore, it is recommended to consult a physician for evaluation and examination.
Doctors often need to take a medical history, perform a physical examination, and conduct tests to establish a diagnosis and treatment plan.

Red spots on the glans (the head of the penis) can be concerning, especially when accompanied by symptoms like itching. It's important to understand that these symptoms can arise from a variety of causes, some of which may require medical attention. Here’s a breakdown of potential issues and considerations based on your description.

1. Irritation or Allergic Reaction: The red spots and itching could be a result of irritation or an allergic reaction. This can happen if there was friction during sexual activity, especially if lubrication was not used. Additionally, if there were any soaps, lotions, or other substances that came into contact with the area, they could cause a localized allergic reaction.

2. Infection: Given that you mentioned unprotected sexual activity, there is a possibility of a sexually transmitted infection (STI). Common STIs such as herpes, gonorrhea, or chlamydia can present with symptoms like redness, itching, or spots on the genital area. It’s crucial to get tested for STIs if you suspect that this might be the case.

3. Balanitis: This is an inflammation of the glans, often caused by poor hygiene, infections, or irritants. Symptoms typically include redness, swelling, and sometimes discharge. If you have a longer foreskin (as you mentioned having a long prepuce), it can sometimes trap moisture and bacteria, leading to balanitis.

4. Fungal Infections: Conditions like candidiasis (yeast infection) can also cause red spots and itching. This is more common in individuals with certain risk factors, such as diabetes or those who are immunocompromised.

5. Dermatological Conditions: Conditions such as psoriasis or eczema can also affect the genital area, leading to red spots and itching. These conditions are usually chronic and may require specific treatments.

6. Phimosis: Since you mentioned that you have a long foreskin, if it is tight and difficult to retract, this condition (phimosis) can lead to irritation and inflammation of the glans, which may present as red spots.

Recommendations:
- Seek Medical Attention: It is advisable to consult a healthcare professional, preferably a urologist or dermatologist, who can perform a physical examination and possibly run tests to determine the exact cause of your symptoms.

- Avoid Irritation: Until you see a doctor, try to avoid any further irritation to the area. This includes avoiding sexual activity, harsh soaps, or any products that could exacerbate the condition.

- Practice Safe Sex: Moving forward, consider using protection during sexual activity to reduce the risk of STIs and other complications.

- Maintain Hygiene: Good hygiene practices are essential, especially if you have a longer foreskin. Regular cleaning can help prevent infections and irritations.

In summary, while red spots on the glans can be due to various benign causes, they can also indicate more serious conditions, especially in the context of recent unprotected sexual activity. It’s important to get a professional evaluation to ensure proper diagnosis and treatment.
