Right nipple
Hello, Dr.
Chou.
I have a small bump on my right nipple that feels a bit painful.
Today is the third day, and I noticed that when I squeezed it, pus and blood came out.
After squeezing, it still feels swollen, and I keep squeezing out more blood (there's quite a bit, and it feels like I can't get it all out).
Could you please tell me what the issue might be?
Xiao Miao, 20~29 year old female. Ask Date: 2016/01/15
Dr. Zhou Jiazhen reply Breast and Thyroid
Dear Xiao Miao: It may be a skin lesion, often caused by infection due to blocked skin glands, which can sometimes lead to the accumulation of pus and recurrent episodes.
If symptoms persist, recur, or enlarge, it is advisable to seek an outpatient examination.

More Info
Hello, thank you for reaching out with your concerns about the painful bump on your right nipple. Based on your description, it sounds like you may be experiencing a condition known as a mammary duct ectasia or possibly a localized infection such as a breast abscess.
Mammary duct ectasia occurs when a milk duct beneath the nipple dilates, fills with fluid, and thickens. This can lead to the formation of a lump that may be painful and can sometimes produce discharge, which can be clear, yellow, or even bloody. This condition is more common in women who are approaching menopause but can occur at any age.
On the other hand, if you are experiencing significant pain, swelling, and discharge that includes blood and pus, it could indicate an infection, such as a breast abscess. An abscess is a collection of pus that can form when bacteria infect the tissue, leading to inflammation and the formation of pus. This condition often requires medical intervention, including antibiotics or possibly drainage of the abscess if it is significant.
Given that you have been experiencing this for three days and have noted the presence of blood and pus, it is crucial to seek medical attention promptly. A healthcare provider will likely perform a physical examination and may recommend imaging studies, such as an ultrasound, to assess the area further. They may also take a sample of the discharge for laboratory analysis to determine if there is an underlying infection and what type of bacteria might be present.
In the meantime, it is essential to avoid squeezing or manipulating the area further, as this can exacerbate the condition and potentially spread any infection. Keeping the area clean and dry is advisable, and you may consider using warm compresses to alleviate discomfort. However, these measures are not a substitute for professional medical evaluation and treatment.
In summary, while there are several potential causes for the painful bump on your nipple, including mammary duct ectasia and breast abscess, the presence of blood and pus is concerning and warrants immediate medical evaluation. Please do not hesitate to contact your healthcare provider or visit an urgent care facility to ensure you receive the appropriate care. Your health and well-being are paramount, and timely intervention can help prevent complications.
