Inquiry about keloid scars?
Hello, doctor.
Last year, I had surgery for a clavicle fracture due to a car accident, and after the surgery, I discovered that I had a 15 cm keloid scar.
This year, I need to undergo another surgery to remove the internal plate.
I would like to know if it is advisable to excise the scar during this surgery.
Additionally, if the scar is removed, will any improvements in my diet (I did not have any dietary restrictions after the first surgery) reduce the likelihood of keloid formation? What dietary considerations should I keep in mind? Thank you!

Dr. Lu Daokuan reply Plastic Surgery
Diet should not be a concern.
During the removal, a consultation with a plastic surgeon can be arranged for scar management.
Afterwards, proper care with artificial skin should be maintained for 3 to 6 months.

When considering the removal of keloid scars during surgery, it is essential to weigh the potential benefits against the risks. Keloids are raised scars that can develop after an injury or surgery, and they often extend beyond the original wound site. They are caused by an overproduction of collagen during the healing process. While surgical removal of keloids is an option, it is important to note that keloids can recur after excision, sometimes even larger than before. Therefore, it is crucial to discuss this with your surgeon, who can evaluate the specific characteristics of your keloid and your overall health to determine the best course of action.
If you decide to proceed with the surgical removal of the keloid, there are several dietary considerations that may aid in the healing process. Although there is no definitive scientific evidence that specific foods can prevent keloid formation, a well-balanced diet can support overall healing and recovery. Here are some dietary tips to consider:
1. Hydration: Staying well-hydrated is crucial for healing. Water helps maintain skin elasticity and supports cellular functions. Aim for at least 8-10 glasses of water a day.
2. Protein-Rich Foods: Protein is essential for tissue repair and regeneration. Incorporate lean meats, fish, eggs, dairy products, legumes, and nuts into your diet to ensure adequate protein intake.
3. Vitamins and Minerals: Certain vitamins and minerals play a vital role in skin health and healing. Vitamin C is important for collagen synthesis, so include citrus fruits, strawberries, bell peppers, and broccoli in your diet. Vitamin E, found in nuts and seeds, may help with skin healing as well. Zinc, present in meats, shellfish, legumes, and seeds, is also crucial for wound healing.
4. Healthy Fats: Omega-3 fatty acids, found in fatty fish, flaxseeds, and walnuts, have anti-inflammatory properties that may benefit skin health. Including these in your diet can support healing.
5. Avoid Processed Foods: Minimize the intake of processed foods high in sugar and unhealthy fats, as they can promote inflammation and may negatively affect healing.
6. Limit Alcohol and Caffeine: Both alcohol and excessive caffeine can dehydrate the body and may interfere with the healing process, so it is advisable to limit their consumption.
7. Anti-Inflammatory Foods: Incorporate foods with anti-inflammatory properties, such as turmeric, ginger, berries, and green leafy vegetables, to help reduce inflammation in the body.
In addition to dietary considerations, it is important to follow your surgeon's post-operative care instructions closely. This may include keeping the surgical site clean, avoiding sun exposure, and possibly using silicone sheets or gels to help minimize scar formation.
Ultimately, the decision to remove a keloid scar should be made in consultation with your healthcare provider, who can provide personalized recommendations based on your medical history and the specifics of your case. If you have concerns about keloid recurrence, your doctor may also discuss adjunct therapies, such as corticosteroid injections or laser treatments, which can help reduce the likelihood of keloid formation after surgery.
