Postoperative pain after sinus surgery?
I would like to ask why there is severe headache starting at night after surgery? It has been three days post-operation, and I have experienced severe pain continuously for three days.
I informed the nursing staff, but they said it is a normal phenomenon due to referred pain.
However, can it really be that severe?

Experiencing severe headaches after sinus surgery can be concerning and may lead to questions about the underlying causes and appropriate responses. It is not uncommon for patients to experience headaches following such procedures, but understanding the reasons behind these symptoms is crucial for effective management and reassurance.
Firstly, it is important to recognize that sinus surgery, which often involves procedures like endoscopic sinus surgery, can lead to inflammation and swelling in the nasal passages and surrounding areas. This inflammation can trigger headaches, particularly if there is pressure build-up in the sinuses. The headaches may be exacerbated at night due to changes in posture or increased pressure in the sinuses when lying down. Additionally, the healing process itself can lead to discomfort as tissues recover and adjust.
The term "referred pain" mentioned by the nursing staff refers to pain that is perceived in a location different from its source. In the case of sinus surgery, irritation or inflammation in the sinus cavities can lead to referred pain in the head, which may be perceived as a severe headache. However, it is essential to differentiate between normal postoperative discomfort and pain that may indicate complications.
Severe and persistent headaches following surgery can also be indicative of other issues, such as cerebrospinal fluid (CSF) leaks, which can occur if there is a breach in the protective layers surrounding the brain. Symptoms of a CSF leak may include severe headaches that worsen when sitting or standing and improve when lying down, along with nausea or vomiting. If you are experiencing these symptoms, it is crucial to seek immediate medical attention, as untreated CSF leaks can lead to serious complications.
Another potential cause of postoperative headaches could be related to tension or muscle strain, particularly if you are experiencing stress or anxiety about your recovery. Tension headaches can manifest as a tight band-like sensation around the head and may be exacerbated by poor posture or muscle tension in the neck and shoulders.
If your headaches have persisted for three days and are described as severe, it is advisable to communicate this to your healthcare provider. While some degree of discomfort is expected after surgery, severe pain that does not respond to over-the-counter pain relief or worsens over time warrants further evaluation. Your doctor may recommend imaging studies, such as a CT scan or MRI, to rule out complications like sinusitis, infection, or structural issues.
In summary, while some degree of headache after sinus surgery can be normal due to inflammation and healing, severe and persistent headaches should not be ignored. It is essential to maintain open communication with your healthcare team, report your symptoms accurately, and seek further evaluation if necessary. Effective management may include pain relief strategies, hydration, rest, and possibly further diagnostic testing to ensure a safe and smooth recovery. Always prioritize your health and well-being by advocating for yourself and seeking help when needed.
