Can thyroid factors affect menstrual cycle irregularities?

Hello, my menstrual cycle has changed to once every two months since June of last year, whereas it used to be regular.
I went to a gynecologist for blood tests, and only my androgen levels were slightly elevated (0.74, with levels above 0.75 considered high), while all other indices were normal.
I recently came across an article stating that the thyroid can also affect the menstrual cycle, and during a previous check-up, my doctor suggested I get my thyroid checked because my neck appeared slightly enlarged.
Can thyroid factors influence abnormal menstrual cycles? Thank you.

Joyce, 20~29 year old female. Ask Date: 2016/01/24

Dr. Zhou Jiazhen reply Breast and Thyroid

Dear Joyce: Thyroid abnormalities can sometimes lead to psychological and emotional changes, which may affect daily life.
Additionally, they can occasionally influence hormonal changes in the body, but generally speaking, the impact is not significant.

Yes, thyroid issues can indeed cause irregular menstrual cycles. The thyroid gland plays a crucial role in regulating various bodily functions, including metabolism, energy levels, and hormonal balance. When the thyroid is not functioning properly, it can lead to a range of symptoms, including menstrual irregularities.

The thyroid hormones, primarily thyroxine (T4) and triiodothyronine (T3), influence the menstrual cycle by affecting the levels of other hormones in the body, such as estrogen and progesterone. An imbalance in thyroid hormones can lead to conditions such as hypothyroidism (underactive thyroid) or hyperthyroidism (overactive thyroid), both of which can disrupt the normal menstrual cycle.

In your case, you mentioned that your menstrual cycle has changed to occurring every two months since June of last year, whereas it used to be regular. This change, along with the elevated androgen levels (male hormones) you noted, could suggest a hormonal imbalance that may be influenced by thyroid function. Elevated testosterone levels can also be associated with conditions such as polycystic ovary syndrome (PCOS), which can further complicate menstrual regularity.

Thyroid dysfunction can lead to various menstrual irregularities, including:
1. Hypothyroidism: This condition can cause heavier, more prolonged menstrual periods (menorrhagia) or even missed periods (amenorrhea). Women with hypothyroidism may experience symptoms such as fatigue, weight gain, and sensitivity to cold, which can also affect their overall health and well-being.

2. Hyperthyroidism: Conversely, an overactive thyroid can lead to lighter, less frequent periods. Women with hyperthyroidism may experience symptoms such as weight loss, increased heart rate, anxiety, and heat intolerance.

Given that you have a family history of thyroid issues and your doctor has suggested checking your thyroid function due to the appearance of your neck, it would be wise to follow up on this recommendation. A simple blood test measuring Thyroid Stimulating Hormone (TSH), T3, and T4 levels can provide valuable information about your thyroid health.
If your thyroid function is found to be abnormal, appropriate treatment can help restore hormonal balance and potentially regulate your menstrual cycle. Treatment options may include thyroid hormone replacement therapy for hypothyroidism or medications to control hyperthyroidism.

In addition to thyroid issues, other factors can also contribute to irregular menstrual cycles, such as stress, significant weight changes, excessive exercise, and underlying medical conditions like PCOS. Therefore, it is essential to consider a comprehensive approach to your health, including lifestyle modifications, stress management, and possibly further evaluation by an endocrinologist or a reproductive endocrinologist.

In summary, yes, thyroid issues can significantly impact menstrual cycles, and it is advisable to investigate your thyroid function further, especially given your symptoms and hormonal levels. Regular monitoring and appropriate management can help address these issues and improve your overall health and menstrual regularity.
