The second menstrual period after an abortion?

Hello, Dr.
Hsu.
I underwent a medical abortion on October 25 of last year (approximately 5 weeks pregnant) and experienced bleeding for two weeks.
During a follow-up appointment, the doctor confirmed that everything was cleared.
A little over a month later, on December 12, I had my first menstrual period (though the flow was very light), and the duration was the same as my previous normal cycles.
My second period also arrived on schedule, but the flow suddenly increased significantly (as I hadn't experienced such a heavy flow for a long time prior to the abortion).
Is this normal?

Dr. Xu Junrui reply Obstetrics and Gynecology

It is indeed common to experience changes in menstrual patterns within three months after undergoing an abortion due to hormonal disruptions in the body.
Therefore, based on your description, this should be considered a normal situation.

After a medication abortion, it is common for individuals to experience changes in their menstrual cycle. The process of a medication abortion typically involves taking two different medications: mifepristone and misoprostol. These medications work together to terminate a pregnancy, and the body may take some time to readjust to its normal hormonal levels afterward.

In your case, you mentioned that you experienced bleeding for two weeks following the abortion, which is typical as the body expels the pregnancy tissue. The first menstrual period you had on December 12, approximately a month after the abortion, is also within the expected timeframe. It is not uncommon for the first few cycles after an abortion to be irregular or different in flow and duration compared to what you experienced prior to the pregnancy.

The increase in menstrual flow you noted during your second period is also a common occurrence. After an abortion, some individuals may experience heavier periods as the body readjusts. This can be due to several factors, including hormonal fluctuations, the body’s response to the recent pregnancy, and the shedding of the uterine lining. It is important to note that while heavier bleeding can be alarming, it is often a normal part of the body’s recovery process.

However, if you find that the bleeding is excessively heavy (for instance, soaking through a pad or tampon every hour for several consecutive hours), or if you experience severe pain, dizziness, or other concerning symptoms, it is crucial to seek medical attention. These could be signs of complications such as retained tissue or other issues that may require further evaluation.

In terms of what to expect moving forward, it is not unusual for menstrual cycles to take a few months to stabilize after an abortion. You may notice variations in flow, duration, and even the timing of your periods. Some individuals may experience more regular cycles after a few months, while others may continue to have irregularities for a longer period.

If you have concerns about your menstrual cycle or if you experience any unusual symptoms, it is advisable to follow up with your healthcare provider. They can provide personalized advice and reassurance based on your specific situation. Additionally, keeping track of your menstrual cycle, including the flow and any associated symptoms, can be helpful information to share with your doctor during follow-up visits.

In summary, while the changes you are experiencing in your menstrual cycle after a medication abortion can be normal, it is essential to monitor your symptoms and seek medical advice if you have any concerns. Your body is adjusting, and it may take some time for your menstrual cycle to return to what you consider normal.
