The discharge is brown with streaks of blood?
Hello Doctor: I would like to ask about my situation.
About three days before my menstrual period, I noticed that my discharge started to be brown.
When my period came, the color was not the usual bright red, but rather a dark red.
After my period ended, the discharge remained brown, and when I wiped after using the restroom, I noticed a light red color on the toilet paper.
I have previously experienced vaginal inflammation, which was resolved after seeing a doctor.
However, I am wondering why this is happening again.
I have an intrauterine device (IUD) in place; could this be the cause?

Dr. Yang Wangjun reply Obstetrics and Gynecology
Hello: Based on your description, it seems like you are experiencing abnormal uterine bleeding.
Typically, a pregnancy test should be conducted first to rule out bleeding due to a miscarriage.
If you are not pregnant, the most likely cause is menstrual irregularity due to anovulation in this cycle.
Additionally, an intrauterine device (IUD) can also cause abnormal shedding of the endometrium leading to bleeding.
These conditions often require hormonal treatment.
Thank you for your message!

Brown discharge with blood before menstruation can be concerning, and it's important to understand the potential causes and implications of these symptoms. The presence of brown discharge, especially when it occurs a few days before your period, is often related to the shedding of old blood that may not have exited the body during your last menstrual cycle. This can be quite common and is usually not a cause for alarm. However, the darker color of your menstrual blood and the presence of light red blood after your period can indicate a few different things.
Firstly, it's essential to consider the role of the intrauterine device (IUD) you mentioned. IUDs can sometimes cause changes in menstrual bleeding patterns, including spotting between periods or changes in the color and consistency of menstrual blood. Some women experience irregular bleeding or spotting, especially in the first few months after insertion. If you have recently had your IUD placed or if it has been a while since you had it checked, it may be worthwhile to consult with your healthcare provider to ensure that it is positioned correctly and functioning as intended.
Additionally, the history of vaginal inflammation you mentioned could also play a role in your current symptoms. Infections or inflammation can lead to changes in discharge, including color and consistency. If you have experienced any other symptoms, such as itching, burning, or unusual odor, it may indicate a recurrent infection that requires medical attention.
Hormonal fluctuations can also contribute to changes in menstrual bleeding and discharge. Stress, changes in weight, or hormonal imbalances can lead to variations in your menstrual cycle. If you have recently experienced significant life changes or stressors, this could be a contributing factor.
It's also important to rule out any other potential causes, such as polyps, fibroids, or other structural abnormalities in the uterus, which can sometimes lead to abnormal bleeding patterns. If your symptoms persist or worsen, or if you experience severe pain, it is crucial to seek medical advice promptly.
In summary, while brown discharge with blood before menstruation can be common and often benign, it is essential to consider your individual circumstances, including the use of an IUD and any previous infections. If you have concerns or if your symptoms change, consulting with a healthcare provider is the best course of action. They can perform a thorough evaluation, including a pelvic exam and possibly imaging studies, to determine the underlying cause and provide appropriate treatment or reassurance.
