Pap smear examination

I had my first sexual intercourse last year.
I know that after having sexual intercourse, I need to get a Pap smear.
I would like to ask if I need to wait three years before getting a Pap smear, or is it recommended that I can go for the Pap smear this year? I have not received the cervical cancer vaccine.
Additionally, I would like to know if getting the vaccine after having sexual intercourse is still beneficial.

Li, 20~29 year old female. Ask Date: 2016/02/08

Dr. Zhang Kunmin reply Obstetrics and Gynecology

Hello, the inquirer: It is recommended that you consult with a gynecologist for an evaluation and arrange for a Pap smear.
You may also consider getting vaccinated to prevent related diseases.
Thank you! Wishing you safety and health.

After your first sexual experience, it is recommended to have a Pap smear within three years, or sooner if you have any concerns or symptoms. The general guideline from the American College of Obstetricians and Gynecologists (ACOG) suggests that women should start cervical cancer screening at age 21, regardless of sexual history. If you are 21 or older and have had sexual intercourse, you should have a Pap smear done. If you are younger than 21, you can wait until you reach that age, but it is advisable to consult with your healthcare provider for personalized recommendations based on your specific circumstances.

Regarding the HPV vaccine, it is still beneficial to receive it even after having sexual intercourse. The HPV vaccine is most effective when given before the onset of sexual activity, but it can still provide protection against strains of the virus that you may not have been exposed to yet. The vaccine protects against the most common high-risk HPV types that cause cervical cancer (such as types 16 and 18) and low-risk types that cause genital warts (such as types 6 and 11).
If you have already had sexual intercourse, it is possible that you may have been exposed to some strains of HPV, but the vaccine can still help protect you from other strains. It is important to note that the vaccine does not treat existing HPV infections or related diseases, but it can prevent new infections. Therefore, getting vaccinated after becoming sexually active is still a good decision.

As for the timing of the Pap smear and the HPV vaccination, there is no strict order that you must follow. You can get the Pap smear and the HPV vaccine at different times, but it is generally advisable to get the vaccine as soon as possible to maximize its effectiveness. If you are due for a Pap smear, you can schedule it alongside your vaccination appointments, but it is not necessary to wait for one before getting the other.

In summary, you should consider scheduling your Pap smear as soon as you are eligible, especially since you have had your first sexual experience. Additionally, getting the HPV vaccine is still beneficial and can provide protection against strains of the virus that you may not have encountered yet. Always consult with your healthcare provider for personalized advice and to discuss any concerns you may have regarding your sexual health and preventive care.
