What are the reasons for a missed menstrual period?

Hello Doctor: My last menstrual period was from January 8 to January 15.
I had sexual intercourse with my boyfriend on January 16, and he used a condom throughout, with ejaculation occurring outside.
However, around January 18 to January 22, I noticed some blood-tinged discharge, and for a few days, it was even clotted.
At that time, I thought it might just be ovulation bleeding, so I didn't pay much attention.
But now, it is February 11, and my period still hasn't arrived, and I am experiencing frequent dizziness.
Could I be pregnant, or is this a symptom of something else? I had symptoms of ovarian dysfunction two years ago; could this be the same issue?

Vanessa, 20~29 year old female. Ask Date: 2016/02/11

Hello, the chances of pregnancy are low.
It is advisable to take proper contraceptive measures before engaging in sexual activity.
If your period does not arrive in another week, it is recommended to see a gynecologist.
Wishing you good health.

When it comes to a late period, there can be a variety of reasons behind it, and understanding these can help you determine the next steps. Based on your description, your last menstrual period was from January 8 to January 15, and you had sexual intercourse on January 16 with protection. You noticed some light spotting or discharge that was blood-tinged from January 18 to January 22, which you attributed to possible ovulation bleeding. Now, as of February 11, your period has not arrived, and you are experiencing dizziness.

Firstly, it’s important to note that a late period can be caused by several factors, including hormonal imbalances, stress, changes in weight, excessive exercise, and underlying health conditions. Given your history of ovarian dysfunction, it is possible that hormonal fluctuations could be contributing to your current situation.
1. Hormonal Imbalances: The menstrual cycle is regulated by a complex interplay of hormones. If there is an imbalance, it can lead to irregularities in your cycle. Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can affect hormone levels and lead to delayed menstruation.

2. Stress: Psychological stress can significantly impact your menstrual cycle. High levels of stress can interfere with the hypothalamus, which regulates the hormones responsible for ovulation and menstruation. If you have been experiencing increased stress, this could be a contributing factor.

3. Ovulation: The spotting you experienced could be ovulation bleeding, which is typically light and occurs around the time of ovulation. However, if your body did not ovulate this cycle (anovulation), it could lead to a delayed period. Anovulation can occur due to various factors, including stress, hormonal imbalances, or significant lifestyle changes.

4. Pregnancy: While you mentioned that you used protection and that the male partner withdrew, no contraceptive method is 100% effective. If there is any doubt, taking a home pregnancy test can provide clarity. It is advisable to wait until at least a week after your expected period for the most accurate result.

5. Other Health Conditions: Conditions such as endometriosis or uterine fibroids can also cause changes in your menstrual cycle. If you have a history of ovarian dysfunction, it may be worth consulting with a healthcare provider to rule out any underlying issues.

Given your symptoms of dizziness, it is essential to consider that this could be related to several factors, including hormonal changes, dehydration, or even anxiety. If the dizziness persists or worsens, it would be prudent to seek medical advice.

In summary, while there are several potential reasons for your late period, including hormonal imbalances, stress, and the possibility of pregnancy, it is crucial to monitor your symptoms closely. If your period does not arrive soon, or if you continue to experience dizziness or other concerning symptoms, I recommend scheduling an appointment with a healthcare provider. They can perform a thorough evaluation, including a physical examination and possibly blood tests, to determine the underlying cause and provide appropriate treatment or reassurance.
