Post-operative issues related to fistula surgery?
Hello Dr.
Ke,
About a week ago, I underwent an anal fistula excision surgery.
I discovered this condition because I felt a hard lump next to my anus.
After the surgery, I applied medication myself and noticed that the hard lump has not disappeared.
Is this a normal phenomenon? If the lump does not go away, how can I determine if the fistula has been completely cleared (since I identified my condition through the hard lump)? Would I need to undergo another surgery? Thank you for taking the time to respond.

After anal fistula surgery, you may feel a "lump" due to the wound not being fully healed.
Whether this is consistent with what you felt before the surgery needs to be confirmed by your surgeon.
Recurrence of anal fistulas is rare within such a short time post-surgery.
Please have your surgeon examine it again for further evaluation.

Post-surgery concerns, especially after a procedure like anal fistula surgery, can be quite daunting. It’s understandable to have questions about recovery and the presence of any lumps or hard areas following surgery.
Firstly, it’s important to recognize that after an anal fistula surgery, some swelling or hard lumps can be a normal part of the healing process. The body goes through various stages of recovery, and inflammation can occur as the tissues heal. However, the persistence of a hard lump, particularly if it was the initial symptom that led to the surgery, warrants careful evaluation.
In your case, the hard lump you are feeling could be a few different things. It might be a residual area of inflammation, scar tissue, or, in some cases, it could indicate that the fistula has not been completely resolved. If the lump remains unchanged or continues to grow, it is essential to consult your surgeon or a healthcare provider for a thorough evaluation. They may perform a physical examination and possibly imaging studies, such as an ultrasound or MRI, to assess the area and determine if there is any remaining fistulous tract or infection.
Regarding the need for further surgery, it depends on the findings during the evaluation. If the hard lump is determined to be scar tissue and there are no signs of infection or remaining fistula, additional surgery may not be necessary. However, if there is evidence of an incomplete fistula or other complications, a follow-up procedure might be required to ensure complete resolution.
In the meantime, it’s crucial to follow your post-operative care instructions closely. This typically includes maintaining good hygiene, managing pain with prescribed medications, and possibly using sitz baths to promote healing. If you experience any signs of infection, such as increased redness, swelling, discharge, or fever, you should seek medical attention promptly.
Additionally, it’s beneficial to keep a record of your symptoms and any changes you notice. This information can be helpful for your healthcare provider in assessing your recovery and determining the best course of action.
In summary, while some hard areas may be normal after anal fistula surgery, persistent lumps should be evaluated by a healthcare professional. Regular follow-ups with your surgeon are essential to monitor your recovery and address any concerns you may have. Don’t hesitate to reach out to your healthcare provider for guidance and reassurance during your recovery process. Your health and peace of mind are paramount, and proactive communication with your medical team is key to a successful recovery.
