Doctor, is it hallux valgus? What should be the treatment?
Hello Doctor,
I would like to ask about my mother's foot.
There is a protrusion at the joint near her big toe, causing what should be a smooth foot shape to bulge outward near the big toe and then curve back in towards the big toe.
Is this what people commonly refer to as hallux valgus? How can I help my mother? She currently has to wear two pairs of socks to avoid friction pain, but she doesn't like it and doesn't want to see a doctor.
Thank you for your assistance.

Based on your description, it appears to be hallux valgus.
If the angle is greater than 30 degrees, there are indications for surgical correction.
If the symptoms are severe, it may compress the second toe, and surgical intervention would be necessary for correction.

More Info
Bunions, medically known as hallux valgus, are a common foot deformity characterized by a bony bump that forms at the base of the big toe. This condition occurs when the big toe deviates towards the second toe, causing the joint at the base of the big toe to protrude outward. The symptoms of bunions can include pain, swelling, redness, and difficulty in wearing shoes. The condition can also lead to the development of corns or calluses due to friction from footwear.
Symptoms of Bunions
1. Visible Bump: The most noticeable symptom is the bony protrusion on the side of the foot at the base of the big toe.
2. Pain and Discomfort: Many individuals experience pain, especially when wearing tight or ill-fitting shoes. The pain can be sharp or achy and may worsen with activity.
3. Swelling and Inflammation: The area around the bunion may become swollen and inflamed, particularly after prolonged standing or walking.
4. Limited Movement: In some cases, the range of motion in the big toe may be reduced, making it difficult to move the toe normally.
5. Changes in Foot Shape: Over time, bunions can alter the shape of the foot, leading to other foot problems such as hammertoes.
Causes of Bunions
Bunions can develop due to a variety of factors, including:
- Genetics: A family history of bunions can increase the likelihood of developing this condition.
- Footwear: Wearing tight, narrow shoes or high heels can contribute to the formation of bunions by forcing the toes into an unnatural position.
- Foot Structure: Certain foot types, such as flat feet or low arches, can predispose individuals to bunions.
- Arthritis: Conditions like rheumatoid arthritis can increase the risk of bunion formation due to joint inflammation.
Treatment Options
For individuals like your mother who are experiencing discomfort from bunions, there are several treatment options available, ranging from conservative measures to surgical intervention:
1. Footwear Modifications: Encourage your mother to wear shoes that provide ample room for the toes. Shoes with a wide toe box can help alleviate pressure on the bunion. Avoiding high heels and tight shoes is crucial.
2. Padding and Orthotics: Using bunion pads or orthotic devices can help cushion the bunion and reduce pain. Custom orthotics may also help in redistributing pressure across the foot.
3. Ice Therapy: Applying ice to the bunion can help reduce swelling and relieve pain. This can be particularly helpful after long periods of standing or walking.
4. Pain Relief Medications: Over-the-counter pain relievers such as ibuprofen or acetaminophen can help manage pain and inflammation.
5. Physical Therapy: Gentle exercises and stretches can improve foot strength and flexibility, potentially alleviating some discomfort associated with bunions.
6. Surgical Options: If conservative treatments fail to provide relief and the bunion causes significant pain or interferes with daily activities, surgical options may be considered. Surgery typically involves realigning the bones of the big toe and removing the bony bump.
Conclusion
It's understandable that your mother may be hesitant to seek medical attention, but it’s important to address her discomfort proactively. Encouraging her to consider non-invasive options first may help her feel more comfortable about managing her condition. If her symptoms persist or worsen, it may be beneficial to consult a podiatrist who specializes in foot conditions. They can provide a thorough evaluation and recommend a tailored treatment plan that suits her needs.
